Coronary artery calcium (CAC) score is a neglected biomarker that can be derived from non-cardiac chest computed tomography scan and represents a surrogate for atherosclerosis. We created a simulation model using different CAC score values in the MESA coronary artery risk score in a population derived from the Fourier Trial. CAC score could modulate the sample sizes of cardiovascular trials in primary and secondary prevention and offer new primary prevention treatments to high-risk subjects with reasonable numbers needed to treat comparable to secondary prevention trials.

Mineralocorticoid receptor antagonists (MRAs) represent one of the cornerstones of treatment for heart failure with reduced ejection fraction. Post-hoc data from the TOPCAT trial, conducted in patients with heart failure mildly reduced or preserved ejection fraction (HFmrEF/HFpEF), suggest the possible clinical benefit of MRAs, particularly for slightly reduced ejection fraction values. The advent of non-steroidal MRAs, including finerenone, seems to represent a turning point in the treatment for HFmrEF/HFpEF.

Background: Late hypertension (HTN) after coarctation of the aorta (CoA) repair contributes to higher morbidity and mortality. An association between transverse aortic arch (TAA) hypoplasia and HTN has been found, but its relationship with surgical strategy is unclear. We studied the association between late HTN and initial surgical strategy pertaining to the TAA.

Ambient Ionization Mass Spectrometry (AI-MS) techniques have revolutionized analytical chemistry by enabling rapid analysis of samples under atmospheric conditions with minimal to no preparation. In this study, the optimization of a cold atmospheric plasma for the analysis of food and pharmaceutical samples, liquid and solid, using a Heat-Assisted Dielectric Barrier Discharge Ionization (HA-DBDI) source is described. A significant enhancement in analyte signals was observed when a heating element was introduced into the design, potentially allowing for greater sensitivity.

Aims: The identification of subjects at higher risk for incident heart failure (HF) with preserved ejection fraction (EF) suitable for more intensive preventive programmes remains challenging. We applied phenomapping to the DAVID-Berg population, comprising subjects with preclinical HF, aiming to refine HF risk stratification.

Methods: The DAVID-Berg study prospectively enrolled 596 asymptomatic outpatients with EF > 40% with hypertension, diabetes mellitus or known cardiovascular disease.

Background: Enhanced recovery after surgery (ERAS) pathways represent a comprehensive approach to optimizing perioperative management and reducing hospital stay and cost. In living donor kidney transplantation, key impediments to postoperative discharge include pain, and opioid associated complications such as nausea, vomiting, and the return of gastrointestinal function.

Methods: In this randomized controlled trial, living kidney transplantation donors were assigned to either the ERAS or control group.

Moderate aortic stenosis is associated with a worse prognosis than milder degrees. Pathophysiologically, this condition in a dysfunctional ventricle could lead to a further mechanism of haemodynamic worsening, so its treatment should lead to clinical advantages for the patient. The low risk of complications associated with percutaneous correction of aortic valve disease (transcatheter aortic valve implantation) should also be considered, which would seem to favour an interventional approach even in the aforementioned condition.

Heart failure with preserved ejection fraction (HFpEF) is a clinical syndrome characterised by the presence of diastolic dysfunction and elevated left ventricular filling pressure, in the setting of a left ventricular ejection fraction of at least 50%. Despite the epidemiological prevalence of HFpEF, a prompt diagnosis is challenging and many uncertainties exist. HFpEF is characterised by different phenotypes driven by various cardiac and non-cardiac comorbidities.

Indoor air quality (IAQ) is critical to the health and wellbeing of people. As the majority of people spend greater amounts of time indoors, either in office spaces or households, the level of air pollutants in such environments is critical. Building materials and furniture are known sources of air pollutants such as Volatile Organic Compounds (VOCs) and may be associated with discomfort, detrimental health of the occupants, etc.

Article Synopsis
  • The study investigates how gender affects heart remodeling and the risk of heart failure (HF) after acute coronary syndrome (ACS), using data from the CORALYS registry.
  • Among 14,699 patients, 31% were women, who had different health profiles compared to men, showing higher rates of hypertension and diabetes but lower rates of smoking and previous heart attacks.
  • Results indicated that women had a higher incidence of both HF hospitalizations and cardiovascular death, establishing female sex as an independent risk factor for these outcomes after ACS.

Myeloproliferative neoplasms, including polycythemia vera, essential thrombocythemia, and myelofibrosis, are characterized by somatic gene mutations in bone marrow stem cells, which trigger an inflammatory response influencing the development of associated cardiovascular complications. In recent years, the same mutations were found in individuals with cardiovascular diseases even in the absence of hematological alterations. These genetic events allow the identification of a new entity called 'clonal hematopoiesis of indeterminate potential' (CHIP), as it was uncertain whether it could evolve toward hematological malignancies.

While the prevalence of heart failure, in general, is similar in men and women, women experience a higher rate of HFpEF compared to HFrEF. Cardiovascular risk factors, parity, estrogen levels, cardiac physiology, and altered response to the immune system may be at the root of this difference. Studies have found that in response to increasing age and hypertension, women experience more concentric left ventricle remodeling, more ventricular and arterial stiffness, and less ventricular dilation compared to men, which predisposes women to developing more diastolic dysfunction.

Background: The right ventricle (RV) has complex geometry and function, with motion along three separate axes-longitudinal, radial, and anteroposterior. Quantitative assessment of RV function by two-dimension echocardiography (2DE) has been limited as a consequence of this complexity, whereas newer three dimensional (3D) analysis offers the potential for more comprehensive assessment of the contributors to RV function. The aims of this study were to quantify the longitudinal, radial and anteroposterior components of global RV function using 3D echocardiography in a cohort of healthy children and to examine maturational changes in these parameters.

The genomic landscape of colorectal cancer (CRC) is shaped by inactivating mutations in tumour suppressors such as APC, and oncogenic mutations such as mutant KRAS. Here we used genetically engineered mouse models, and multimodal mass spectrometry-based metabolomics to study the impact of common genetic drivers of CRC on the metabolic landscape of the intestine. We show that untargeted metabolic profiling can be applied to stratify intestinal tissues according to underlying genetic alterations, and use mass spectrometry imaging to identify tumour, stromal and normal adjacent tissues.
