Gluten Sensitivity Underlying Resistant "Laryngopharyngeal Reflux" Symptoms and Signs.

Introduction: Laryngopharyngeal reflux (LPR) is one of the most common conditions encountered in otolaryngology. Gluten sensitivity may mimic the signs and symptoms of LPR or act as an aggravating cofactor with LPR. Gluten sensitivity and food intolerance also have been implicated as conditions that may be associated specifically with LPR symptoms and signs resistant to traditional medical treatment.

Safety and Efficacy of Outpatinet Type 1 Thyroplasty Using Gore-Tex for Mobile Vocal Folds.

Introduction: Type 1 thyroplasty treats glottic insufficiency (GI) through vocal fold medialization. The safety of type 1 thyroplasty and its efficacy in an outpatient setting have not been investigated in patients with mobile vocal folds.

Objective: The purpose of this study was to investigate efficacy and safety of outpatient type 1 thyroplasty using Gore-Tex for mobile vocal folds.

5-Fluorouracil for Treatment of Vocal Fold Scar.

Background: Vocal fold (VF) scar can result from trauma, neoplasm, inflammatory processes, congenital causes, surgery and other etiologies. In general, once the vibratory margin of the VF has been scarred, it has not been possible to return VF function to normal; but often it can be improved. The drug 5-fluorouracil (5-FU) is a pyrimidine antimetabolic that has many clinical applications ranging from systemic chemotherapy to topical treatment of actinic keratosis and basal cell carcinoma of the skin.

Thyroarytenoid Muscle Avulsion.

Thyroarytenoid muscle avulsion is a rare condition in which laryngeal trauma causes a separation of the thyroarytenoid muscle from the arytenoid cartilage. Typically, symptoms are nonspecific but include severe dysphonia and voice fatigue. They are similar to symptoms of vocal process avulsion.

Unresectable Primary Tracheal Synovial Sarcoma.

Synovial sarcoma (SS) comprises less than 1% of head and neck cancers, and less than five cases of adult primary tracheal SS have been described. This case describes a patient encountered at a community-based academic hospital, and retrospective chart review was performed for data collection. A woman in her forties presented with shortness of breath due to a superior mediastinal mass found to be an unresectable primary tracheal SS.

Correlation Between Presbylarynx and Laryngeal EMG.

Background: The normal aging process affects many human functions profoundly. The voice is no exception, and some aging effects on the voice are obvious including vocal fold atrophy, bowing, stiffness, more prominent vocal fold processes, and glottic insufficiency. The study's primary aim was to determine the correlation between presbylarynx and laryngeal electromyography (LEMG) results and to compare these electromyography (EMG) results with young and old voice patients without evidence of presbylarynx changes on strobovideolaryngoscopy.

Incidence of and Risk Factors Associated With Vocal Fold Hemorrhage Following Type I Thyroplasty With Gore-Tex Implant.

Objectives: The objectives of this study were to determine the rate of hemorrhage following type I thyroplasty performed exclusively with Gore-Tex implant and to define whether age, comorbidities, anticoagulation therapy, presence of vocal fold ectasia, and operative technique are associated with increased incidence of hemorrhage.

Methods: Medical charts of 86 patients who underwent type I thyroplasty with Gore-Tex implant between the years 2013 and 2019 were reviewed retrospectively and divided into two groups based on presence or absence of postoperative vocal fold hemorrhage. Patients were examined on the day following surgery.

Effects of Particle Size of Inhaled Corticosteroid on the Voice.

Objectives: The purpose of this study was to determine if inhaled corticosteroid (ICS) particle size influences the development of laryngitis including candida laryngitis, dysphonia, or vocalis muscle atrophy in asthmatic patients.

Study Design: Retrospective analysis.

Methods: Medical records of patients from a quaternary care laryngology practice who have asthma were reviewed retrospectively.

Assessing the sharpness of hypodermic needles after repeated use.

Four gauges (14G, 16G, 18G, 20G) of single use hypodermic needles were evaluated for sharpness by measuring the force required to puncture rehydrated bovine leather. The needles began to dull after 1 use with maximum bluntness occurring within 4 to 5 uses.

Biomechanical and performance implications of weapon design: comparison of bullpup and conventional configurations.

Objective: Shooter accuracy and stability were monitored while firing two bullpup and two conventional configuration rifles of the same caliber in order to determine if one style of weapon results in superior performance.

Background: Considerable debate exists among police and military professionals regarding the differences between conventional configuration weapons, where the magazine and action are located ahead of the trigger, and bullpup configuration, where they are located behind the trigger (closer to the user). To date, no published research has attempted to evaluate this question from a physical ergonomics standpoint, and the knowledge that one style might improve stability or result in superior performance is of interest to countless military, law enforcement, and industry experts.

The neurological emergency room and prehospital stroke alert: the whole is greater than the sum of its parts.

Background: Emergency medical services (EMS) prenotification to hospitals regarding the arrival of patients who have had a stroke is recommended to facilitate the workup once the patient arrives. Most hospitals have the patient enter the emergency department (ED) before obtaining a head computed tomography (CT) scan. At Capital Health, prehospital stroke-alert patients are delivered directly to CT and met by a neurological emergency team.

Longitudinal trends in subcutaneous fat thickness during adolescence.

Longitudinal principal components (LPC) analysis is used to summarize adolescent trends in subcutaneous fat thickness at three sites (biceps, suprailiac, and abdominal). For each site, component 1 (C1) represents fatness level, and component 2 (C2) represents an increase followed by a decrease (fat wave) in fatness. Components 3-5 (retained for girls only) represent more complicated patterns of change (multiple increases and decreases).

Early adult antecedents of life satisfaction at age 70.

Association between Life Satisfaction Ratings at about age 70 and cognitive, personality, interpersonal, and family characteristics in early adulthood are examined using data from the Guidance Study at the University of California, Berkeley. When the parents of participants in that study were in their early 30s, the mothers were rated on 15 cognitive and personality characteristics and both parents were rated on personal, interpersonal, and family variables. Approximately 40 years later, the surviving parents were interviewed intensively and assigned Life Satisfaction Ratings, For both sexes, certain traits of their own at 30 are correlated with life satisfaction at 70.
