Three-dimensional echocardiography for predicting postoperative ventricular volumes after surgical ventricular reconstruction of left ventricular aneurysm: A case-based presentation.

Three-dimensional transthoracic echocardiography (3DTTE) may have a role in predicting final left ventricular volumes and clinical response after the surgical ventricular reconstruction (SVR) of left ventricular aneurysms and pseudoaneurysms. Left ventricle final volumes can be calculated through "virtual aneurysmectomy." We present a patient with a huge ventricular dilation combined with myocardial dissection, localized wall rupture, and aneurysm of the left ventricular apex after acute myocardial infarction in which accurate predictions from 3DTTE suggest potential clinical value.

[Clopidogrel versus prasugrel in acute coronary syndrome treated with coronary angioplasty].

Greater antithrombotic potency new antiplatelet agents have been added such as prasugrel (PR) and ticagrelor to the traditional use of clopidogrel (CL) in the treatment of acute coronary syndrome (ACS). This study was aimed at comparing the incidence of long term ischemic and hemorrhagic events in patients treated with CL or PR during hospitalization. Retrospective ACS data base analysis performed by our cardiology service was completed prospectively.

[Improvement in functional capacity after levothyroxine treatment in patients with chronic heart failure and subclinical hypothyroidism].

Aim: To assess whether levothyroxine treatment improves functional capacity in patients with chronic heart failure (New York Heart Association class i-iii) and subclinical hypothyroidism.

Methods: One hundred and sixty-three outpatients with stable chronic heart failure followed up for at least 6 months were enrolled. A physical examination was performed, and laboratory tests including thyroid hormone levels, Doppler echocardiogram, radionuclide ventriculography, and Holter monitoring were requested.

[QTc interval prolongation and polymorphic ventricular tachycardia related to subarachnoid hemorrhage].

A 55-yr-old woman was taken to the hospital after recovering from a presyncopal episode. The electrocardiogram showed sinus bradycardia with QTc interval of 840 msec. Few minutes later, the patient developed a polymorphic ventricular tachycardia and subsequent cardiac arrest requiring cardiopulmonary resuscitation.
