Active Life: a project for a safe hospital-community transition after arthroplasty.

Objective: To define the criteria for the continuity of care to elderly people submitted to arthroplasty.

Method: This is a qualitative study, inserted in the constructivist paradigm, whose methodological option fell on research-action. The participants were the health professionals of an orthopedic service and of the community care teams in the area of the hospital.
