These clinical trials show the efficiency of therapy with angiotensin-converting enzyme (ACE) inhibitors in essential hypertension (EH), chronic heart failure and coronary heart disease (CHD). The SMILE studies have indicated positive results of the early administration (<24 hours) of zofenopril in patients with acute myocardial infarction (AMI). These results have been confirmed in high-risk patients, including those with hypertension and diabetes mellitus, which enables zofenopril to be recommended for clinical use in patients with EH and CHD.

Together with haemodynamic disorders activation of inflammatory reactions, which involve anti-inflammatory cytokines, is shown to be one of the main factors affecting CHF course and prognosis. Alteration of cytokine level is associated with injury level, gravity of the disease course and its clinical outcome.

The proposed scheme of treatment reduced number of recurrences, maintained normal sinus rhythm and allowed reaching relatively rapid normalization of mechanical function of left atrium.

For this reason 62 patients from 37 to 70 years old (mean age 54, 6 +/- 6,1) with CHF (class II-III NYHA) and persistent AF underwent the medical treatment. The suggested scheme of treatment improved HF functional class by NYHA, raised anti reactive effectiveness and favorably influenced on the diastolic function of the LV in this type of patients.
