The classical manifestations of primary hyperparathyroidism (hypercalcemia, hypophosphatemia, hypercalciuria, increased blood alkaline phosphatase level, and bone changes of the type of generalized osteoporosis) were found in approximately half of patients in a group of 23 who were examined. When Recklinghausen's disease is suspected, the patients must be subjected to a purposeful examination. The intraoperative diagnosis of parathyroid adenoma is difficult.

Based on an analysis of results of 213 operations of total gastrectomy and resection of the cardia the authors made a conclusion that the method of connection of the esophagus with the intestine (stomach) depended on the specific situation and must not be standard. Such differentiated tactics of choice of the access and methods of anastomosing during proximal resection of the stomach and gastrectomy resulted in twice less amount of complications and lethality resulting from incompetence of the anastomosis suture.

The authors have analyzed the interrelationship between postoperative complications and surgical approach in the treatment of carcinoma of the proximal portion of the stomach. The radical operations were carried out upon 140 patients; in 93 of them the transperitoneal approach was applied and in 47-the transpleural one. It has been found that in the transpleural approach the insufficiency of anastomosis sutures and postoperative pancreatitis occur much more seldom, and bronchopulmonary complications rate the first.
