Recommendations from a Canadian Delphi consensus study on best practice for optimal referral and appropriate management of severe asthma.

Background: In Canada, severe asthma affects an estimated 5-10% of people with asthma and is associated with frequent exacerbations, poor symptom control and significant morbidity from the disease itself, as well as the high dose inhaled, and systemic steroids used to treat it. Significant heterogeneity exists in service structure and patient access to severe asthma care, including access to biologic treatments. There appears to be over-reliance on short-acting beta agonists and frequent oral corticosteroid use, two indicators of uncontrolled asthma which can indicate undiagnosed or suboptimally treated severe asthma.

Understanding the Potential Benefits of Cannabidiol for Patients With Schizophrenia: A Narrative Review.

Research suggests that cannabis-derived delta-9-tetrahydrocannabinol can be linked to the worsening of psychosis and/or other symptoms of schizophrenia. However, studies have shown that another major cannabinoid found in cannabis, cannabidiol (CBD), may be a potential alternative or adjunctive treatment for psychosis and schizophrenia. As such, herein we review the relevant literature relating to the safety and efficacy of CBD treatment in patients with schizophrenia, including the effects of CBD in treating the positive, negative, and cognitive symptoms of the disorder, as well as the molecular mechanisms by which CBD can reduce schizophrenic symptoms.

Development and Validation of Early Warning Criteria to Identify Escalated Care Events in Neonatal Intensive Care Unit Patients.

Objective: This study aimed to identify and validate the diagnostic utility of a set of clinical and laboratory criteria (early warning criteria [EWC]) that portend a clinical deterioration event (escalated care event [ECE]) in neonatal intensive care unit (NICU) patients.

Study Design: Using the RAND appropriateness method, we first established a consensus on seven ECE, that is, events that require additional monitoring, treatment, or stay in the NICU or that were associated with morbidity. We then established consensus on EWC that could portend an ECE from an initial set of 32 potential EWC items to a final set of 10 items.

The Effects of Resveratrol in Patients with Cardiovascular Disease and Heart Failure: A Narrative Review.

Cardiovascular disease (CVD) is the main cause of death globally and responsible for the second highest number of deaths in Canada. Medical advancements in the treatment of CVD have led to patients living longer with CVD but often progressing to another condition called heart failure (HF). As a result, HF has emerged in the last decade as a major medical concern.

Costs of maternal health care services in three anglophone African countries.

This paper is a synthesis of a case study of provider and consumer costs, along with selected quality indicators, for six maternal health services provided at one public hospital, one mission hospital, one public health centre and one mission centre, in Uganda, Malawi and Ghana. The study examines the costs of providing the services in a selected number of facilities in order to examine the reasons behind cost differences, assess the efficiency of service delivery, and determine whether management improvements might achieve cost savings without hurting quality. This assessment is important to African countries with ambitious goals for improving maternal health but scarce public health resources and limited government budgets.

Visual word recognition in bilinguals: evidence from masked phonological priming.

Bilingual written language representation was investigated with the masked phonological priming paradigm. Pseudohomophonic and control primes of French target words were used to show that Dutch-French bilinguals exhibit the same pattern of phonological and orthographic priming as native French speakers, which suggests that the same processes underlie first-and second-language processing. It was also found that for bilinguals, but not monolinguals, it is possible to prime a target word of the second language with a homophonic stimulus (either word or nonword) of the first language.

Management of geriatric behavior problems.

Agitation has many causes, and we have no way of classifying it that dictates the choice of the right agents or methods of treatment. It is also difficult to judge the effectiveness of any method. Because agitation is an intermittent phenomenon one can seldom be certain if improvement or worsening is merely incidental to the form of treatment instituted.

Gait kinematics in below-knee child amputees: a force plate analysis.

A complete gait analysis was performed on six children with below-knee amputations to determine the kinematics and kinetics of amputee gait in children. A relatively new method of analysis--the inverse dynamic relationship--was used to determine the joint moments in the sagittal plane of the joints in the intact limb and the remaining portion of the amputated limb. The joint moments in the sagittal plane of the intact limb in the child amputee were found to be normal or below normal during level walking.

The appearance of oestrone sulphate in the peripheral plasma of the pig early in pregnancy.

Detectable concentrations of oestrone sulphate were present in 50% of the plasma samples collected from pregnant animals by Day 17. No oestrone sulphate was detected in plasma from cyclic nonpregnant pigs.
