Most countries in oriental Asia have not yet experienced the 'western' coronary heart disease (CHD) epidemic despite substantial economic development. An exception has been Singapore. We compared mortality and CHD risk factors in Singapore with two Oriental locations, Hong Kong and mainland China, which have not experienced the CHD epidemic.

Case-control study of lung cancer during 1994-1997 in the birth cohort in Tasmania, Australia, with an excess of female cases during 1983-1992.

Objective: In Tasmania, Australia, lung cancer incidence for 25-44-year-old women has reached that of 25-44-year-old men despite less smoking by the women. We investigated whether this could be due to greater female-than-male relative risk for smoking.

Methods: This was a case-control study of lung cancer in the 1939-1964 birth cohort.

An association between plastic mattress covers and sheepskin underbedding use in infancy and house dust mite sensitization in childhood: a prospective study.

Background: Higher house dust mite (HDM) allergen exposure during infancy has been associated with increased HDM sensitization. Infant bedding has been associated with the accumulation of varying levels of HDM. Prospective data on the relationship between infant bedding and the development of HDM sensitization has not been previously examined.

The use of mutually exclusive categories for atopic sensitization: a contrasting effect for family size on house dust mite sensitization compared with ryegrass sensitization.

Our aim was to examine the relative importance of family size on sensitization to two different allergens: ryegrass and house dust mite (HDM), using a mutually exclusive classification for allergen-specific sensitization. An 8-year follow-up birth cohort study of children born between 1988-89 was conducted. The follow-up sample consisted of 498 children residing in Northern Tasmania in 1997 (84% of eligible).

Parental smoking and infant respiratory infection: how important is not smoking in the same room with the baby?

Objectives: We sought to quantify the effect of good smoking hygiene on infant risk of respiratory tract infection in the first 12 months of life.

Methods: A cohort of 4486 infants in Tasmania, Australia, was followed from birth to 12 months of age for hospitalization with respiratory infection. Case ascertainment was 98.

The need for a new advanced nursing practice role for Australian adult critical care settings.

The current deficit of knowledge related to advanced nursing practice in Australian adult critical care settings presents a barrier to future role development. This paper reports the findings of one theme identified by a research study that explored issues related to the potential for a new advanced nursing practice role for these settings. The qualitative approach of focus group methodology was used to collect data from a purposive sample of regional Queensland critical care nurses (n = 26).

A randomized controlled trial of phytoestrogen supplementation, growth and bone turnover in adolescent males.

The purposes of this review are fourfold. 1) We summarize a theoretical basis for classifying all possible steady-state inner medullary countercurrent concentrating mechanisms based on mass balance principles.

The renal pelvis: machinery that concentrates urine in the papilla.

Two decades ago, Bodil Schmidt-Nielsen and Bruce Graves documented the rhythmic contractions of the renal pelvis in a remarkable video, visually demonstrating how peristaltic waves empty the papilla and how the subsequent elastic recoil draws water from the collecting duct and into the tethered-open ascending vasa recta. Thus a periodic hydrostatic gradient generates an axial osmotic gradient. This review recapitulates the video and offers a link to figures and scenes digitized from the original tape.

Synthetic bedding and wheeze in childhood.

Background: The reasons for the increase in childhood asthma over time are unclear. The indoor environment is of particular concern. An adverse role for synthetic bedding on asthma development in childhood has been suggested by cross-sectional studies that have found an association between synthetic pillow use and childhood wheeze.

Syndrome X in 8-y-old Australian children: stronger associations with current body fatness than with infant size or growth.

Objective: Syndrome X (clustering of insulin resistance, dyslipidaemia and hypertension) in adults with central obesity has been suggested to be a consequence of poor foetal development. We investigated clustering of syndrome X factors in a sample of 8-y-old Australian children, and whether the clusters were associated with size at birth and childhood obesity.

Design: Longitudinal, 1997 follow-up of children enrolled as singleton-born neonates in 1989.

Quantifying the change of melanoma incidence by Breslow thickness.

Melanoma incidence has increased throughout the world over the past 25 years. A surrogate for the severity of melanoma is the Breslow thickness of the lesions. Data on melanoma, including Breslow thickness, were collected in 1978-1980 and 1988-1990 from the Tasmania Tumor Registry.

Population trends in sudden infant death syndrome.

This review documents and assesses recent trends in sudden infant death syndrome. We review medical literature, Internet resources, and national governmental data. A striking reduction in SIDS incidence of more than 50% has been observed in various countries after interventions, particularly during the early 1990s, to reduce the prevalence of prone infant sleeping.

Misclassification due to body hair and seasonal variation on melanin density estimates for skin type using spectrophotometry.

Recent advances have enabled quite accurate estimations of cutaneous melanin density by spectrophotometry using reflectance of light at wavelengths 400 and 420 nm. Our purpose was to assess the effect of body hair and seasonal variation at the upper inner arm and buttock on measurements of melanin density. We estimated melanin density of 104 volunteers at 3-monthly intervals over 12 months both before and after shaving.

Is birthweight a good marker for gestational exposures that increase the risk of adult disease?

There is much evidence of a link between low birthweight and elevated risk of adult cardiovascular disease, from humans and experimental animals. However, if one relies on data linking birthweight to coronary heart disease to estimate the public health implications of this association, the effects are likely to be modest. The focus on birthweight may be misplaced, because reduced size at birth may not be in the causal pathway linking gestational factors to disease in adult offspring.

Feather bedding and house dust mite sensitization and airway disease in childhood.

Feather bedding has been inversely associated with child wheeze and also with house dust mite (HDM) allergen levels. We conducted a cross-sectional analysis of the childhood component of a birth cohort study. Our aim was to examine the relation between feather bedding and HDM sensitization and airway disease.

The rising prevalence of diabetes and impaired glucose tolerance: the Australian Diabetes, Obesity and Lifestyle Study.

Objective: To determine the population-based prevalence of diabetes and other categories of glucose intolerance (impaired glucose tolerance [IGT] and impaired fasting glucose [IFG]) in Australia and to compare the prevalence with previous Australian data.

Research Design And Methods: A national sample involving 11,247 participants aged > or =25 years living in 42 randomly selected areas from the six states and the Northern Territory were examined in a cross-sectional survey using the 75-g oral glucose tolerance test to assess fasting and 2-h plasma glucose concentrations. The World Health Organization diagnostic criteria were used to determine the prevalence of abnormal glucose tolerance.

Genetic dissection of the human leukocyte antigen region by use of haplotypes of Tasmanians with multiple sclerosis.

Association of multiple sclerosis (MS) with the human leukocyte antigen (HLA) class II haplotype DRB1*1501-DQB1*0602 is the most consistently replicated finding of genetic studies of the disease. However, the high level of linkage disequilibrium (LD) in the HLA region has hindered the identification of other loci that single-marker tests for association are unlikely to resolve. In order to address this issue, we generated haplotypes spanning 14.

Cutaneous melanin density of Caucasians measured by spectrophotometry and risk of malignant melanoma, basal cell carcinoma, and squamous cell carcinoma of the skin.

Recent advances have enabled quite accurate estimation by spectrophotometry of the density of cutaneous melanin. The relation between skin cancers and this objective measure of skin phenotype is examined here. For this purpose, a population-based case-control study of subjects aged 20-59 years of northern European ancestry was conducted in Tasmania, Australia.

Lung cancer incidence in Australia: impact of filter-tip cigarettes with unchanged tar yields.

Filter-tip cigarettes became popular in Australia in the late 1950s, but "tar" yields remained high for another decade. Because of this, the effect of filters independently of tar reductions can be estimated by comparing the age-adjusted incidence of lung cancer for relevant birth cohorts of Australians. Separate analyses by histologic type may throw some light on the specific effects of filters.

Alternate substrates of human glutaryl-CoA dehydrogenase: structure and reactivity of substrates, and identification of a novel 2-enoyl-CoA product.

The dehydrogenation reaction catalyzed by human glutaryl-CoA dehydrogenase was investigated using a series of alternate substrates. These substrates have various substituents at the gamma position in place of the carboxylate of the physiological substrate, glutaryl-CoA. The steady-state kinetic constants of the six alternate substrates and the extent of flavin reduction in the anaerobic half-reaction were determined.

Nurses' behaviour regarding CPR and the theories of reasoned action and planned behaviour.

Cardiopulmonary resuscitation (CPR) has been used in hospitals for approximately 40 years. Nurses are generally the first responders to a cardiac arrest and initiate basic life support while waiting for the advanced cardiac life support team to arrive. Speed and competence of the first responder are factors contributing to the initial survival of a person following a cardiac arrest.
