Chronic Suppurative Otitis Media Patient Presenting With Hyperhomocysteinemia in Granulomatosis With Polyangiitis.

Granulomatosis with Polyangiitis (GPA) can present with Cerebral Venous Sinus Thrombosis (CVST), Chronic Suppurative Otitis Media, and Lower Motor Neuron (LMN) Facial Palsy. However, an association between CVST and Hyperhomocysteinemia in GPA has not previously been reported. Here, we report a case of CVST and Hyperhomocysteinemia in Proteinase 3 anti-neutrophil cytoplasmic antibody (PR3-ANCA) positive GPA without renal involvement.

Assessment of Serum Minerals in Subclinical Hypothyroid and Overt Hypothyroid Patients.

Background Hypothyroidism, the commonest form of hormonal dysfunction, is due to thyroid hormone deficiency or its impaired activity. Homeostasis of the metabolism of minerals is regulated by thyroid hormones. If there is any disorder of the thyroid it will lead to disturbances of metabolism of minerals.

Prevalence of Dengue Serotypes and Its Correlation With the Laboratory Profile at a Tertiary Care Hospital in Northwestern India.

Background and aim Dengue fever is an emerging arboviral public health problem in a large endemic population in the tropical and sub-tropical areas of the world, with varying degrees of clinical presentation. This study was aimed at analyzing the clinical and laboratory dynamics of the four dengue serotypes. Methods This institutional review board (IRB)-approved hospital-based observational study was performed with 100 in-patients with dengue infection above 12 years of age, without co-morbidities or known malignancy, in a tertiary care center in Northern India.

A Case Report on Acute Pancreatitis in a Patient With Coronavirus Disease 2019 (COVID-19) Pneumonia.

With the ongoing coronavirus disease 2019 (COVID-19) pandemic, there has been an explosion of scientific literature on the clinical manifestations and pathogenesis of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Gastrointestinal symptoms occur in 15-20% of COVID-19 patients; however, there have not been many case reports on acute pancreatitis in COVID-19 patients. The expression of ACE-2 ([angiotensin-converting enzyme 2] the host receptor for SARS-CoV-2) is very high in the pancreas, which might be a contributing factor, but the high expression is mainly localized to endocrine pancreas.

Pulmonary Embolism Mimicking Acute Severe Asthma.

When a patient presents with wheezing, pulmonary embolism is not usually considered as a possible cause. However bronchoconstriction can be caused by the embolism which produces wheezing that may be so obvious to make a diagnosis of bronchial asthma. Here we present a case of sub-massive pulmonary embolism presenting predominantly with wheezing without any previous history of cardio pulmonary diseases.

Transverse myelitis in antiphospholipid antibody negative systemic lupus erythematosus.

Acute transverse myelitis is a well known neurological complication occurring in systemic lupus erythematosus. Many prior studies have shown a link between transverse myelitis and the presence of antiphospholipid antibodies. Earlier theories have linked thrombotic tendency to be the culprit in such manifestations but currently there is evidence to support other causative mechanisms.

Revisiting plummer vinson syndrome.

Plummer Vinson syndrome is a rare association of postcricoid dysphagia, upper esophageal webs, and iron deficiency anemia. Iron deficiency state has been hypothesized to play an etiological role. While literature review elucidates the resolution of dysphagia in most cases with iron therapy, we discuss our case where the dysphagia was resistant to such therapy and necessitated a mechanical dilatation.

Paraphenylenediamine: Blackening more than just hair.

Paraphenylenediamine is an important constituent of hair dye toxicity of which one could herald fatal complications such as rhabdomyolysis, renal failure, angioneurotic edema, and respiratory failure. We present a case of hair dye poisoning that presented with respiratory distress due to laryngeal edema and later developed trismus, subclinical tetany, apnea, and conduction abnormality on electrocardiogram. This case report highlights the need for a thorough toxicological review of the components of any ingested substance.

ATT- A Double Edged Sword?

Antitubercular therapy (ATT) induced hepatotoxicity, although well known to clinicians, is often over looked and underrated. Given the low threshold of starting ATT, especially empirically, the adverse manifestations can take a considerable toll. A variety of associated risk factors compound the morbidity.

Revisiting endosulfan.

Endosulfan toxicity could precipitate gargantuan jeopardy and may result in irreversible and fatal damage. The spectrum of involvement may range from mild nausea, vomiting, and anxiety to intractable seizures and multiorgan damage resulting in death. We report a case of endosulfan poisoning complicated by multi-organ dysfunction, cardiac arrest, and death.

ARDS following inhalation of hydrochloric acid.

The clinical spectrum of Inhalation injury can range from mild cough to a devastating ARDS. We herewith present a patient who is a goldsmith by occupation and his work consists of dissolving gold in Hydrochloric acid. He had accidentally inhaled fumes of Hydrochloric acid and presented with cough and breathlessness, later on required mechanical ventilation for ARDS and improved.
