While occupation began as the central concept of occupational therapy at the dawn of the profession, the use of occupation throughout the therapy process, including assessment, has wavered in recent decades. Occupation is defined as meaningful and purposeful activities that individuals do to occupy their time and attention. The use of occupation-based assessments is vital to establishing care with an occupational focus.

Background: The effectiveness of occupation-based hand therapy and the barriers to the use of occupation-based interventions (OBIs) have been established, but the current experience of hand therapists using OBIs and the extent of the use of OBIs in practice is unknown.

Objective: This study aimed to identify the frequency that occupational therapists who are Certified Hand Therapists (CHTs) utilized OBIs, how occupational therapists who are CHTs described their application of OBIs, and identify the supports and barriers to the application of OBIs.

Methods: Twenty-nine participants completed a survey that included 27 questions.

ABSTRACT This mixed methods survey examines the perceptions of occupational therapists working in hand therapy in the United States regarding the benefits of and barriers to occupation-based hand therapy. Electronic questionnaires containing open- and closed-ended questions were answered by 105 occupational therapists working in hand therapy and analyzed using descriptive statistics and qualitatively according to themes. Participants reported that occupation-based interventions benefit clients by facilitating functional activity, meaningful therapeutic experiences, and holistic approach to treatment.
