Hypercoagulable disorders can compromise success of free flap reconstruction. Factor V Leiden is one such disorder for which only one previously reported case of successful free tissue transfer in the head and neck has been described. We report a 70-year-old woman with factor V Leiden treated for stage IVA squamous cell carcinoma of the mandible with a composite resection and reconstruction with an osteocutaneous scapular free tissue transfer.

Maxillofacial subunit reconstruction using vascularized fibula free flap and endosseous implants is a complex and exciting topic. Use of this technique has profoundly improved patients' function, form, and quality of life. This article outlines the goals and requirements of reconstruction and patient selection.

Soft tissue reconstruction of head and neck ablative defects is a broad, challenging, and subjective topic. The authors outline goals to keep in mind when deciding on a primary reconstructive option for defects created by oncologic resection. Factors considered in local, regional, and distant flap selection are discussed.

Background/aim: Hydrogen sulfide (HS) and the enzymes that synthesize it, cystathionine-b-synthase, cystathionine γ-lyase, and 3-mercaptopyruvate, are increased in different human malignancies. Due to its short half-life, HS concentrations have not been directly measured in a human malignancy. Here we directly measured in vivo HS levels within oral squamous cell carcinoma (OSCC).

Oral cancer results in significant morbidity and mortality in those afflicted. Despite modern treatments, mortality is high, with overall 5-year survival rates that approximate 50%. However, early stage disease (Stages 1 and 2) carries a significantly better prognosis than late stage disease (Stages 3 and 4), therefore, prevention and early detection continue to be important in improving oral cancer control.

Maxillary reconstruction.

Oral Maxillofac Surg Clin North Am

Postablative maxillary defects present a wide range of functional and esthetic challenges. Several classification schemes have added clarity to the subject, but the surgeon must maintain a clear vision of the defect and appreciate its reconstructive implications. Local tissue flaps remain valuable tools in the reconstruction of small isolated defects of the posterior maxilla and palate; however, microvascular free flaps have eclipsed prosthetic obturators as the mainstay of therapy in advanced postablative defects of the maxilla.
