The 2018 Social Security Financing Act introduced, in its Article 51, several experiments and innovations in health care, including the Free, Responsible and Supportive Nursing Teams initiative. The aim is to offer patients holistic care focused on their empowerment and quality of life rather than on the production of technical acts.

[The Équilibres experiment, a paradigm shift in home care].

It was in 2019 that the Equilibres nursing teams began to be deployed in three metropolitan regions: Hauts-de-France, Île-de-France and Occitanie. This organizational experiment, authorized by the Article 51 of the Social Insurance Financing Law, targets home nursing and mobilizes caregivers committed to patient autonomy and responsible regarding public health expenses.

[Step 1 screening: experimentation in Occitania with nurses and pharmacists].

Screening with the step 1 tool is the principal modality of entry into the ICOPE program. This preliminary step is particularly an initiative of primary care health professionals, who are daily caring older people. In Occitania, as part of the deployment of ICOPE program, nurses and pharmacists, were invited to integrate the step 1 in their practices.

[Doctor-nurse cooperation in a therapeutic education pathway at home].

A therapeutic education programme comprising individual sessions in the patient's home has been tried out with patients with diabetes. It enables them to gain a better understanding of the disease and become players in their own health care. This initiative strengthens the doctor-nurse partnership within a coordinated care pathway.
