Clinical Reasoning: A 62-Year-Old Woman With Progressive Spasticity, Weakness, and Gait Instability.

Evaluating patients with adult-onset gait instability can pose diagnostic challenges. In this case, a 62-year-old Ashkenazi Jewish woman with a history of surgically corrected hammer toes presented with chronic progressive lower extremity weakness, sensory impairment, and gait instability. Subsequently, the patient developed urinary urgency and mild cognitive difficulties.

Neurosyphilis presenting with Guillain-Barre syndrome: a case report.

Background: Syphilis is associated with a wide variety of systemic presentations, earning it the moniker "The great mimicker". Neurosyphilis is classically associated with meningovasculitis in the acute-subacute stage and tabes dorsalis and dementia paralytica in later stages. However, one of the less well described presentations include Guillain-Barre Syndrome.

COVID-19-associated AMPA-R and CRMP-5 autoimmune encephalitis in a patient with thymoma and myasthenia gravis.

Thymomas are associated with autoimmune disease, most commonly myasthenia gravis, and rarely with autoimmune encephalitis. More recently, viral triggers including COVID-19 have also been implicated in autoimmunity. We present a case of antibody-positive autoimmune encephalitis that developed in the setting of COVID-19 in a patient with thymomatous myasthenia gravis.
