Based on the follow-up of 1,600 patients with sarcoidosis from 1971 to 1996, the authors analyzed the value of various methods for identifying the disease, the frequency and nature of misdiagnoses. They showed the efficiency of basic treatments in patients with sarcoidosis: corticosteroidal hormones, nonhormonal antiinflammatory drugs, plasmapheresis, and physiotherapy. Four cases of Sjogren's disease are reported: 2 cases with pulmonary syndrome (fibrosing alveolitis) and 2 cases with respiratory sarcoidosis. The diagnosis was made basing on xerostomia and xerophthalmia. Corticosteroid treatment produced only a partial response.

Intricate questions related to the differential diagnosis of diverse diffuse pulmonary disorders are analysed in more than 1500 patients. Among them 1100 patients had sarcoidosis, 284 alveolitis of different etiology (including 62 patients with fibrotic alveolitis of autoimmune genesis), 45 rare pulmonary diseases, 5 carcinomatosis and 40 disseminated tuberculosis. 70% of the patients were hospitalized for pulmonary tuberculosis.

The following 3 forms of an unfavorable course of sarcoidosis were identified and described: 1) infiltrative-and-pneumonic; 2) that taking the course of fibrotic alveolitis; and 3) generalized sarcoidosis with hepatolienal syndrome.
