We reported a case of kaposiform angiodermatitis (Bluefarb-Stewart syndrome) complicating a superficial arterio-venous malformation of the skin and the sub-cutaneous tissue of the right trochanter area of a 28 year-old-man. This lesion resulted from large arterio-venous shunts occurring over a one-year period inside the vascular malformation, which remained stable for 27 years. This case report underlines that any vascular malformation may take an uncertain turn and needs an attentive follow-up with adequate treatment.

An inadvertent puncture of the brachio-cubital artery during an intravenous perfusion provoked a false aneurysm in a 59-year-old woman. The angiographic check-up disclosed a "pile of dishes" pattern on the humeral artery. Pathological examination of the resected false aneurysm confirmed the presence of an extensive medial fibromuscular dysplasia distant from the rupture area.

When the left anterior descending coronary artery follows an anomalous course between the aorta and pulmonary artery it can cause myocardial ischaemia or sudden death during exercise in young people. Coronary arteriography in a 27 year old man with angina pectoris at rest showed a left anterior descending coronary artery arising from a common right trunk and running from the aorta to the pulmonary artery. Follow up after revascularisation was uneventful.

A 54 year old man, hospitalised for thoraco-abdominal pain resulting from a septicemia which gives positive hemocultures for streptococcus D Bovis, is diagnosed to have a splenic abscess which will require splenectomy. At the same time, an endocarditis develops and gets worse, with auriculo-ventricular blockade and, especially, major aortic insufficiency, which is the cause of death by a brutal and massive pulmonary oedema. In the progression of an endocarditis, the occurrence of a splenic abscess, primary localisation of the initial septicemia or the secondary of an arterial septic embolism, is a rare contingency compared to the frequency of splenomegaly or splenic infarction: less than 2 percent of the cases in the literature.

Two cases of interventricular septal rupture (VSR) in elderly patients (71 and 74 years) occurring at the 5th and 12th day of primary postero-inferior myocardial infarction, were reported. The diagnosis was made at 2D echocardiography and confirmed at catheterisation with coronary angiography. Good surgical results were obtained after operation 72 and 12 hours after VSR (9th and 12th infarct days, respectively): the first patient had a good outcome with a 2 year follow-up, but the second patient died in the 55th postoperative day, of renal failure.

The authors report a case of sclerosing cholangitis following surgical treatment of a hydatid cyst of the liver. The review of the literature demonstrates the rarity of this serious complication. The authors speculate on the mechanism of this disease: postoperative infection of the common bile duct, or chemical trauma associated with a cysto-biliary fistula.

In reviewing a series of twelve cases treated surgically for deep venous thrombosis, with an admittedly short follow up (2 months to 2 years), the authors stress the beneficial effects of venous surgery. The postoperative mortality rate was zero and the morbidity rate marginal. They underline the importance of phlebographic exploration leading to earlier diagnosis, and list the therapeutic indications which vary according to the case and may include a supporting heparinotherapy and fibrinolytic treatment in a specialized center.
