Surgical management of chronic fistula after sleeve gastrectomy.

Background: There is no clear definition of the chronic leak after sleeve gastrectomy. There are several endoscopic approaches, including endoprothese, endoscopic clips, endoscopic sealing glue, or balloon dilation. In case of failure of the endoscopic treatment, a definitive surgical approach can be attempted.

Capture of viable circulating tumor cells in the liver of colorectal cancer patients.

Background: The incidence and number of circulating tumor cells (CTCs) in the peripheral blood of colorectal cancer patients are lower than in other cancer types, which may point to a particular biology of colorectal cancer affecting CTC detection.

Methods: We detected CTCs in the peripheral and mesenteric blood of colorectal cancer patients by use of 2 independent technologies on the basis of different biological properties of colon cancer cells. Seventy-five patients diagnosed with localized (M0, n = 60) and metastatic (M1, n = 15) colorectal cancer were included.

Abdominal wall incisional and inguinal hernia repair can call for utilization of implants or prostheses as an alternative to simple suture techniques. The various implants can be synthetic, biologic or mixed: their physicochemical properties condition the mechanical results and the long-term outcome of the repair. The increasing number of available materials allows the surgeon to choose between a wide variety depending on the indication, the site of implantation, the surgical approach and whether the operative field is contaminated or not.

Introduction: The prevention or the management of digestive fistulae may be performed by using an external wrap of collagen of animal origin. To evaluate this treatment, an experimental study creating a hole in the colon of pig covered by a resorbable collagen belt was performed. Results are very interesting and collagen wrap is very well tolerated by the colon wall.

[A case of intrahepatic gastrinoma].

The Zollinger-Ellison syndrome is due to an endocrine gastrin-secreting tumor, the gastrinoma. This tumor is often malignant and patients develop metastases in 25% of cases. The usual localizations of gastrinomas are at the head of the pancreas, the duodenal wall and the peripancreatic lymph nodes.

[Ileocecal intussusception secondary to a cecal endometriosis].

We report the case of a 43-year-old woman, who presented a complete and non-reductible ileo-cecal intussusception with pre-occlusion. An ileocecal resection with ileocolic anastomosis was performed. The pathologic examination confirmed the diagnosis of colic endometriosis with transmural lesions, causing the intussusception.

[Endocrine tumor arising in heterotopic gastric pancreas].

We report the case of a 49-year-old caucasian woman, in whom an endocrine tumor arising in gastric heterotopic pancreas was diagnosed. The patient was treated surgically with a gastric wedge resection. Heterotopic pancreas is a benign anatomic condition, probably widely underdiagnosed because usually asymptomatic.

Background: To maintain the long-term effects of a gastric bariatric operation, bands are often placed to control the restriction. Erosion into the gastric wall by these devices remains a problem. A soft resiliant prosthesis of animal origin, constituted by a network of non-absorbable collagen fibres, may be a solution to this problem.

Concurrent aberrant right gastric vein directly draining into the liver and variations of the hepatic artery.

We report an autopsy on a 46-year-old man, a case that presented the concurrence of two rare vascular variations of the lesser omentum: aberrant right gastric vein draining directly into the liver, and multiple hepatic arteries. Although the left gastric vein emptied into the left aspect of the portal vein, the right one was found to ascend from the gastric lesser curvature along the right aspect of the common bile duct and to reach directly the porta hepatis. A left hepatic artery originating from the left gastric artery entered the porta hepatis in conjunction with the left ramus of the portal vein.

We report on an autopsy in which both right and left gastric veins drained directly into the liver without joining the portal vein.
