We present а rare clinical case of successful surgical treatment - combined left-sided hemicolectomy and cecal resection for colon cancer of two locations in a patient with idiopathic thrombocytopenic purpura, which, in our opinion, is due to radiation exposure is presented. It is shown that the biological effects of long-term ionizing radiation on critical organs can have a diverse nature both in terms of the time of occurrence and in their structure, which requires an individual approach, the involvement of related specialists and the necessary medical preoperative preparation for precise planning of surgical intervention taking into account all possible risks.

Despite current research in the development of drugbased cytostatic and targeted therapies in the treatment of breast cancer, radiation therapy (RT) is an important component in the treatment of this pathology in the postoperative period. At the same time, an important problem is the development of pathological changes in the heart in the longterm period, which worsen the quality of life and increase the risk of cardiac death 10 or more years after RT The problem of heart damage due to RT is gaining new significance due to the existing increase in the number of cancer patients requiring radiation exposure in the modern world. RT of malignant tumors of the breast and organs of the chest cavity causes damage to all structures of the heart, including the coronary arteries, valves, conducting system, pericardium.

MEKOC microscopy complexes have a group of specialized automatic functions for medical analyses of biomaterials integrated with general virtual microscopy accessories. Such functions provide a way of making specialized reference virtual slides (RVS). The latter contain the results of virtual analysis or expert evidence of the automatic analysis results presented in the virtual slide.

Characteristics of modern equipment components for general- or special-purpose modular systems of automated microscopy are considered. Medical, technical, and economic assessment of various configurations of automated microscopy systems is performed. These systems can be used in telemedicine and hematological, cytological, and parasitologic analysis.

The paper presents the results of medical tests of a group of computer-aided procedures for microscopic analysis by means of a MECOS-Ts2 complex (ZAO "MECOS", Russia), which have been conducted at the Republican Children's Clinical Hospital, the Research Institute of Emergency Pediatric Surgery and Traumatology, and Moscow City Clinical Hospital No. 23. Computer-aided procedures for calculating the differential count and for analyzing the morphology of red blood cells were tested on blood smears from a total of 443 patients and donors, computer-aided calculation of the count of reticulocytes was tested on 318 smears.

Computer-aided and information systems of a MECOS-Ts2 microscopy complex designed to transmit a representative image of a specimen along communication lines are considered. The above systems allow the use of the complex: 1) in on-line and off-line modes to provide tele-medical services (cytology, histology, etc.); moreover, the requirements for channel traffic may be decreased over and over again as compared to the routine on-line videomicroscopy mode; 2) for telemedical tests of specialized local automatic procedures of microscopical analysis (analysis of smears of blood and bone-marrow smears, parasitological analysis, etc.

Disadvantages of conventional procedures for testing of medical automated microscopy systems are considered. A testing procedure based on step-by-step testing of the system units is suggested. The procedure makes use of remote data analysis through communication lines.

A clinical example of effective treatment of an adult patient with combined deformations of the jaws (upper prognathism of the third degree and open occlusion of the second degree) is reported. Fragmented osteotomy of the maxilla at the level of symmetrical removal of second molars is performed. Due to Engle's arches soldered with Vasil'ev's splint and compression, a maxillary fragment was shifted 10 mm backward and downward at the expense of compressing the inter-root septae; the first molars were preserved.

The authors have generalized the experience of treatment of 82 injured with injuries of condylar processus fracture treatment of the lower jaw. By a surgical way were successfully eliminated posttraumatic ankylosis and contracture of temporomandibular articulations with use of titanium endoprosthesis and semi-joints. The osteosynthesis of condylar processus of the lower jaw by titanium miniplates and screws was also practiced.

The article analyses experience in the treatment of 81 patients with diffuse polyposis who underwent subtotal resection of the colon with abdominoanal resection of the rectum and downward displacement of the right parts into the anal canal. It is shown that growth of the remaining occasional polyps and appearance of new polyps are encountered in the maintained parts of the colon and upper parts of the gastrointestinal tract in the postoperative period. The authors found that endoscopic polypectomy allows the right colon to be preserved in 92.

The endoscopic sanitation is possible in patients operated upon for diffuse polyposis in cases of the absence of malignization of the polyps in the other parts of the colon. The endoscopic polypectomy performed on patients with diffuse polyposis allows preserving the rectum in 88.8% and the upper parts of the gastrointestinal tract in 99.

The authors make an analysis of treatment of 87 patients with closed unstable fractures of diaphysis of the femoral bone with the application of their two devices for immersional and extrafocal osteosynthesis. In all 87 patients consolidation of the fracture was achieved. Results of the treatment were recognized as good in 84 patients (96.

In patients with proliferative and mixed forms of total polyposis of the digestive tract the intestinal polyps are mainly localized in the duodenum, with hamartomal polyposis--in the jejunum and ileus. The endoscopic polypectomy from the small intestine may be performed intraoperatively and during endoscopy under narcosis. 174 polyps with the diameter of 0.

Under consideration are results of treatment of fresh closed diaphyseal fractures of the forearm bones in 313 patients, 172 of them were given conservative treatment and 141 were treated surgically. It was shown that if the fractures can be set by closed means and fragments can be held in the correct position by a plaster bandage this method gives favorable outcomes in a considerably greater amount of cases than surgical methods. It allows the extremity function to be completely preserved in almost half of the patients.
