Investigations designed to study gastric mucosa (GM) in patients with chronic primary gastroduodenitis with the aid of electron microscopy showed the abnormal changes in the cellular, subcellular membranes and capillaries to be of the same type as they are in peptic ulcer (PU) though poorly manifest. Changes in GM under chronic atrophic gastroduodenitis (ChGD) suggested involutive changes while in chronic secondary gastroduodenitis (ChSGD) it was generally not different from the mucosa ultrastructure in healthy subjects except for changes in surface epithelium. Based on the histoenzymologic studies it has been ascertained that in ChPGD there occurs a rise in lysosomal enzymes similar to that encountered in PU, reflecting the intensity of catabolic processes, decrease in the activity of enzymes of the capillary wall and mitochondria respiratory chain against the background of compensatory enhancement of the activity of the glycolytic enzyme lactate dehydrogenase.

A peroral sugar-reducing preparation gliquidone (glurenorm) was tried in treatment of 66 patients with digestive diseases (peptic ulcer, chronic pancreatitis, cirrhosis of the liver), presenting with diabetes mellitus. The results of the studies made showed glurenorm to be a highly effective preparation in term of both its sugar-reducing effect and its capability to improve energy processes in gastric and duodenal mucosa, pancreacytes, hepatocytes, besides which, it appear to dispel metabolic disturbances and stimulate reparative processes in body organs, improves protein-synthetic processes in the liver, pancreas, mucosa of the gastroduodenal region.

Tardipherone is capable of maximum absorption in the initial portion of small intestine, iron being in its retarding form, which fact ensures a prolonged action of the drug, mucoprotease being a part of tardipherone composition acting to guard gastric and duodenal mucosae against irritating effect of iron. Tardipherone was found to be efficacious in treating patients with ulcer disease complicated by bleeding. Under its effect the hemogram indices get normalized as does the ratio of aggressive to protective factors of gastric juice, which factors play a major part in the emergent relapse of the illness.

Based on the studies made in 84 patients with calculous cholecystitis and 12 patients with calcifying pancreatitis, an efficiency was shown of infusional administration of pyrencepine or ranitidine to prevent stress postoperative erosive-and-ulcerous lesions of gastroduodenal zone. Gastrocepine is shown to be superior to ranitidine and can be withdrawn without a maintenance dose, as well as becouse its cytoprotective effects with respect of the gastric and duodenal mucosa.

A study made in 72 patients showed a positive influence in ulcer disease of gastrocepine and H2-blockers of receptors to histamine on histoenzymological and ultrastructural changes in the mucous membrane of the stomach and duodenum. Gastrocepine was found to be superior to H2-blockers of receptors to histamine due to its apparent cytoprotective effect and ability to improve microcirculation.

The indices for the gastric contents aggression and protection ratio returned to normal in patients with peptic ulcer and preulcerous condition during the treatment with smekta due to a decrease in proteolytic activity and increase in the synthesis of the mucus protective proteins. Monitoring of intragastric pH in the body of the stomach throughout 24 hours in patients with peptic ulcer of duodenal location revealed significant acid-neutralizing effect of smekta. Patients with nonspecific ulcerous colitis enjoyed freedom of side effects as a result of the use of salofalk and derived full therapeutic benefit from this treatment.

Effectiveness of the use of Omeprazolum (Oside) was studied in 24 patients with ulcer disease, 16 patients with chronic primary gastroduodenitis and 18 patients with acid reflux-esophagitis. The preparation Oside is a highly effective agent for the treatment of the aforementioned conditions and possesses a powerful antisecretory and antioxidant activity. Against the background of the use of the preparation the count of Helicobacter pylori gets decreased in the antral part of the stomach.

In the present investigation on the treatment of patients with nonulcerous dyspepsia and peptic ulcer, hiatus hernia complicated with reflux esophagitis, a mechanism has been studied of the action of topalkan, a 3rd generation aluminium-magnesium antacid, that involves not only its antacid properties but in an even greater degree the availability of alginic acid in its formula which has antipeptic and protective effect with respect of the mucous membrane of the stomach and esophagus, and provides, together with the magnesium salts, a floating layer of the preparation in the stomach. In grave course of nonulcerous dyspepsia (preulcerous condition) and peptic ulcer complicated by reflux disease of the esophagus topalkan was used in combination with present-day antisecretory preparations pyrenzepine or H2-blockaders of histamine receptors capable of enhancing the antisecretory effect of topalkan as well as endowed with cytoprotective properties with respect of the mucous membrane of the stomach and duodenum promoting healing of erosions and healing and scarring of ulcerous defects.

Intravascular laser radiation of the blood was employed in 68 patients with ulcer disease in the complex treatment with antacid; cholinolytic, antioxidant agents. The employment of helium-neon laser as an immunomodulator resulted in an improvement of indices of cellular and humoral immunity, normalization of lipid peroxidation, increased resistance of the gastric and duodenal mucosa, reduction of the time of scarring of the ulcer defect against the background of rapid clinical remission, increase of the interrecurrence period.

The study of 468 ulcer patients treated by laser irradiation plus choline blocking agents with adjuvant alpha-tocopherol antioxidants (alpha-tocopherol acetate, wild rose and sea-buckthorn oils) provided evidence in favour of the antioxidants addition. Combination of hyperbaric oxygenation (5-6 sessions) or riboxin with choline blocking agents made antioxidants unnecessary. Biochemical findings (vitamin E and malonic dialdehyde concentrations), electron microscopic, ++histo-enzyme and amino acids studies supported the clinical evidence.

The following method of examination of patients with chronic primary gastroduodenitis is proposed: 1) clinical examination; 2) roentgen and endoscopic examination; 3) assessment of genetic predisposition to ulcer disease; 4) examination of the acid- and pepsin-forming gastric function; 5) determination of the pepsin aggression ratio; 6) determination of the gastric juice defense ratio; 7) biochemical examination of the amino acid composition of the gastric mucosa; 8) determination of the lipid peroxidation intensity. The obtained data indicate that chronic primary gastroduodenitis is not a pre-disease but an initial stage of ulcer disease.

Dioxidine++-induced changes were shown to occur in the protein composition of the cells of Staphylococcus aureus. The most significant damages were observed in the composition of exoproteins. Dioxidine++ had a specific inhibitory effect on intracellular nuclease, which was accompanied by a decrease in virulence and disorders in the toxin formation.

For screening substances with antiviral activity the possible use of the influenza virus neuraminidase was studied on fluoren as an example. Functional and structural changes of the hepato-biliary system were assessed in 56 persons with different forms of chronic alcoholism. In patients with prenosological forms of chronic alcoholism despite the absence of clinical manifestations and functional disorders of the liver, phenomena of fatty infiltration of the liver were revealed by means of ultrasound examination. Patients with chronic alcoholism of grade II and III showed an increase of the activity of aminotransferases, increased values of the thymol test, hypoalbuminemia, disorders of lipid metabolism.

The authors established the necessity that during laser therapy of patients with ulcer disease this treatment should be associated with the use of cholinolytic agents and antioxidants of the alpha-tocopherol type. The content of fibrin fibrinogen splitting products (FSP), radioimmune trypsin, C-peptide and carbohydrate antigen (CA) 19-9 in the blood of 82 patients with acute pancreatitis (edematous and hemorrhagic), and chronic recurrent pancreatitis at the stage of exacerbation, 42 patients with chronic pancreatitis, 34 patients with cancer of the pancreas (stages III-IV) and 22 healthy persons were studied. Results indicate a high diagnostic value of determination FSP, trypsin and C-peptide in patients with acute pancreatitis and chronic recurring pancreatitis at the stage of exacerbation, trypsin and C-peptide in patients with chronic pancreatitis associated with severe exocrine insufficiency of the pancreas, KA 19-9 in patients with cancer of the pancreas.

Experience in the use of CT in combined radiodiagnosis of pneumonia was analysed. It has been concluded that CT objectively reflects morphological inflammatory pulmonary changes and permits their all-round assessment over time. The diagnosis of pneumonia in CT is based on classical x-ray symptoms.
