Article Synopsis
  • The study aimed to compare pregabalin and gabapentin as pre-emptive analgesics for patients undergoing elective lumbar spine surgeries.
  • Both medications were administered one hour before anesthesia, and their effectiveness was measured by the time until the first rescue analgesic dose was needed and postoperative pain levels.
  • Results showed that pregabalin provided longer pain relief and reduced the need for additional analgesics compared to gabapentin, indicating that it might be a better choice for managing postoperative pain.

Background: Many adjuvants have been used with local anesthetics to reduce the time of onset and prolong the duration of analgesia in brachial plexus blocks. However, few studies are there using verapamil as an adjuvant with levobupivacaine.

Aims: This study aims to study the effects of verapamil as adjuvant to levobupivacaine in supraclavicular block for upper extremity surgery.

Article Synopsis
  • Dexamethasone (8 mg) added to levobupivacaine for supraclavicular brachial plexus block significantly enhances analgesic effects, extending both sensory and motor block durations.
  • In a study of 60 patients, those receiving dexamethasone experienced longer times before needing additional pain relief (>700 mins) compared to the control group receiving saline (~396 mins).
  • The findings suggest that combining dexamethasone with levobupivacaine improves pain management in surgical settings by reducing the need for rescue analgesics and speeding up the onset of the block.

Although transversus abdominis plane (TAP) block is an effective way of providing analgesia in post-operative abdominal surgery patients; however, it can be considered as an anesthetic technique in high-risk cases for surgery. We report a case of a geriatric female with chronic obstructive pulmonary disease in the respiratory failure, hypotension, posted in an emergency with old perforation leading to peritonitis. The surgery was successfully conducted under bilateral TAP block, which was used as a sole anesthetic technique.
