Medical management of gastroesophageal reflux disease.

Gastroesophageal reflux disease is a common worldwide disorder. Most patients have typical symptoms of heartburn, regurgitation and exacerbation of symptoms with large meal volumes, fatty meals, recumbency or bending forward. Patients who show typical symptoms for under 10 years may be treated empirically with lifestyle and dietary modifications plus acid-suppressing medications.

Glucagon for the relief of esophageal food impaction does it really work?

We sought to evaluate our experience with glucagon used in the emergency department setting to relieve esophageal food impaction (EFI). We reviewed the records of patients with food impaction who received glucagon between January 1998 and May 2003 and recorded patient demographics, medical history, symptoms following glucagon administration, and endoscopic findings. There were 92 episodes of food impaction in 85 patients.

Enhanced magnification-directed biopsies do not increase the detection of intestinal metaplasia in patients with GERD.

Background: The diagnosis of Barrett's esophagus (BE) requires histologic confirmation of specialized intestinal metaplasia (SIM) through biopsy, a technique prone to sampling error. One method designed to improve the yield of biopsy uses acetic acid with magnification endoscopy: enhanced magnification endoscopy (EME). This technique identifies several mucosal surface patterns, and of these, pattern types III and IV have been associated with SIM.

Dysphagia.

Dysphagia is a common complaint that always warrants investigation. The patient's history and preliminary testing can help differentiate between the two types of dysphagia: oropharyngeal or esophageal. Specific treatments for either of these types of dysphagia depend on the underlying etiology.
