Article Synopsis
  • Recent epidemiological data suggest that more males than females with COVID-19 experience severe outcomes like ICU admission and death, although this isn't universally accepted.
  • Proposed reasons for this trend include higher smoking rates in males, testosterone deficiency leading to inflammation, protective effects of estrogen in females, and genetic factors affecting immune response.
  • The review analyzes current evidence from studies to evaluate these hypotheses, finding strong support for testosterone deficiency and cytokine errors, while also noting the complex nature of the findings regarding estrogen's role and questioning the impact of smoking and androgen-driven mechanisms.

Introduction: Testosterone prescriptions have increased dramatically in recent years, largely because of changes in expert guidelines. Concerns have been raised that testosterone therapy (TTh) may be associated with an increased incidence of conditions such as cardiovascular (CV) disease, thromboembolic events, obstructive sleep apnea (OSA), benign prostatic hyperplasia (BPH), and prostate cancer (PCa) and also may be a beneficial therapy in the management of prediabetes. As such, considerable debate remains regarding which hypogonadal populations are appropriate candidates for TTh.
