Article Synopsis
  • The study aimed to identify anesthetic agents that reduce negative outcomes during laryngeal mask airway (LMA) insertion in adult patients receiving general anesthesia.
  • A network meta-analysis of 53 randomized controlled trials involving 4695 patients found that the combination of propofol and dexmedetomidine was most effective in minimizing adverse events during the procedure.
  • Overall, all anesthetic combinations, except for those involving thiopental, were better than propofol alone in reducing adverse outcomes related to apnea, insertion failure, and inadequate anesthesia depth.

Cognitive behaviour therapy for psychosis (CBTp) should be offered to patients receiving psychiatric inpatient care, yet very little is known about patients' perspectives on this. The aim of this study was to examine patients' experiences of a CBTp-informed intervention delivered in inpatient settings. We recruited 10 participants from the intervention arm of a randomised controlled trial examining the feasibility and acceptability of a CBTp-informed intervention for psychiatric inpatient settings.

The incorporation of phytoactive compounds in the management of malarial vectors holds promise for the development of innovative and efficient alternatives. Nevertheless, the molecular and physiological responses that these bioactive substances induce remain underexplored. This present study investigated the toxicity of different concentrations of aqueous and methanol extracts of Ocimum tenuiflorum against larvae of Anopheles gambiae (sensu stricto) and unraveled the possible underlying molecular pathways responsible for the observed physiological effects.

Introduction: Zimbabwe has a smoking prevalence of 11.7% among the adult population (15 years and older). Thus, in the absence of effective tobacco control measures, the economic burden of tobacco use will be aggravated, especially considering the increasing tobacco industry activity in the country.

There is some evidence of differences in psychosis care provision by ethnicity. We investigated variations in the receipt of Cognitive Behavioural Therapy for psychosis (CBTp) and family intervention across ethnic groups in Early Intervention in Psychosis (EIP) teams throughout England, where national policy mandates offering these interventions to all. We included data on 29,610 service users from the National Clinical Audit of Psychosis (NCAP), collected between 2018 and 2021.

Article Synopsis
  • Microscopic unilateral laminotomy for bilateral decompression (ULBD) is a minimally invasive surgery for lumbar spinal stenosis that may offer advantages like reduced instability and improved outcomes compared to traditional laminectomy (CL).
  • A scoping review covering studies from 1990 to 2022 found 17 relevant articles, with different research designs, including randomized trials and retrospective analyses, examining the effects of ULBD.
  • The findings suggest ULBD might result in shorter surgery times and less blood loss while maintaining similar clinical outcomes compared to CL, but the limited and varying quality of the studies makes it hard to draw definitive conclusions.

Objective: To investigate if COVID-19 UK lockdown measures resulted in a delay in the presentation and treatment of patients with cauda equina syndrome (CES).

Methods: This is a multicenter retrospective study of patients with surgically treated CES across 3 time periods: April-May 2020 (first lockdown), August-September 2020 (no-lockdown group), and January-February 2021 (second lockdown). Data regarding duration of symptoms, time from referral to admission, time from admission to surgery, and postoperative outcomes were collected.

Article Synopsis
  • - Scoliosis in children is a common spinal deformity that may lead to worsening aesthetics and could require surgical intervention, especially when conservative treatments fail; sometimes, it's linked with intramedullary tumors, complicating management.
  • - The study details a surgical method that combines titanium implants for initial correction of scoliosis in two adolescents with tumors, which are later swapped for carbon fiber implants to preserve the correction and enable better post-op imaging.
  • - The proposed technique is noted for being safe, reliable, and effective, allowing for improved MRI evaluations post-surgery while minimizing metal-related imaging issues.

Background: Inpatient psychiatric care is unpopular and expensive, and development and evaluation of alternatives is a long-standing policy and research priority around the world. In England, the three main models documented over the past fifty years (teams offering crisis assessment and treatment at home; acute day units; and residential crisis services in the community) have recently been augmented by several new service models. These are intended to enhance choice and flexibility within catchment area acute care systems, but remain largely undocumented in the research literature.

Background: Sports-related injuries are the third commonest cause of spine fractures. Spinal fractures incurred as a result of partaking in sport by their nature are different from those associated with frailty and road traffic accidents. The patient demographics and nature of fractures associated with sports activities are not well documented.

Article Synopsis
  • The study investigates how the COVID-19 pandemic has uniquely affected individuals with pre-existing mental health conditions, highlighting their personal experiences and perceptions during this time.
  • Through qualitative interviews with 49 participants conducted by researchers with lived mental health experiences, the research identifies exacerbated mental health issues, challenges in social connectivity, and varying access to mental health services.
  • The findings emphasize the need for effective and equitable mental health care solutions, particularly remote options, and call for further research into the long-term effects of the pandemic, especially for marginalized BAME communities.

Purpose: Paediatric spine trauma is uncommon and is managed differently from adults due to the anatomical differences of the paediatric spine. The paediatric spine is less ossified, with lax ligaments and a higher fulcrum in the c-spine which results in a different pattern of injuries. The aim of this study is to provide a contemporary audit of paediatric spinal trauma.

Background: People with serious mental illness (SMI) have sexual health needs but there is little evidence to inform effective interventions to address them. In fact, there are few studies that have addressed this topic for people with SMI outside USA and Brazil. Therefore, the aim of the study was to establish the acceptability and feasibility of a trial of a sexual health promotion intervention for people with SMI in the UK.
