Article Synopsis
  • Ulcerative proctitis (UP) is a rare form of ulcerative colitis in children, with the study analyzing clinical features and outcomes of 196 diagnosed patients aged under 18 from 2016 to 2020.
  • Most patients presented with symptoms like bloody stools and abdominal pain, and the study found that higher initial disease activity scores (PUCAI) were linked to worse outcomes, including the need for more aggressive treatments.
  • By the end of the study, nearly half of the patients showed disease progression, and only a small percentage required colectomy, highlighting the challenges in managing UP in pediatric patients.

Objective/background: Endoscopic balloon dilatation (EBD) has been shown to be effective and safe in adults with stricturing Crohn disease (CD) yet pediatric data is sparse. We aimed to assess efficacy and safety of EBD in stricturing pediatric CD.

Methods: International collaboration included 11 centers from Europe, Canada, and Israel.

Background: Maintaining of remission early in the disease course of Crohn's disease (CD) is essential and has major impact on the future prognosis. This study aimed to identify baseline predictors to develop model allowing stratification of patients who will not benefit from long-term azathioprine (AZA) treatment and will require more intensive therapy.

Methods: This study was designed to develop clinical prediction rule using retrospective data analysis of pediatric CD patients included in prospective inception cohort.

Unlabelled: The ability of medical centers in Eastern and South-Eastern Europe to diagnose and treat fungal infections remains unknown. In order to investigate that, here we conducted a cross-sectional online survey, released at both The International Society for Human & Animal Mycology (ISHAM) and European Confederation of Medical Mycology (ECMM) websites. A total of 31 institutions responded to the questionnaire.

Objectives: Adult studies suggest that patients with isolated colonic Crohn disease (L2 CD) exhibit unique characteristics differentiating them from patients with ileo-caecal (L1) CD and ulcerative colitis (UC). We aimed to characterize clinical features and outcomes of paediatric patients with L2.

Methods: Retrospective data was collected through the Porto Inflammatory Bowel Disease group of the European Society for Paediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) on Paediatric patients with L2, L1 or UC at different time-points.

Aim: To evaluate the value of abdominal ultrasonography (US) in the follow-up of paediatric patients with ulcerative colitis (UC) compared to faecal calprotectin (FC) and colonoscopy.

Material And Method: In this retrospective study we enrolled 30 paediatric patients previously diagnosed with UC, examined by abdominal US and colonoscopy within the same week. FC was also determined during the same week.

Background: Assessing the inflammation is important in the follow-up of paediatric patients with inflammatory bowel disease (IBD). We aim to evaluate the value of B cell-activating factor (BAFF) in paediatric IBD as a potential biomarker for follow-up.

Method: We determined BAFF in serum and faeces and faecal calprotectin (CP) in 32 IBD children-16 Crohn's disease (CD) and 16 ulcerative colitis (UC).

Article Synopsis
  • The study investigates vaccination rates and immunization status in children with inflammatory bowel disease (IBD) across two groups based on diagnosis timing (before and after June 2012).
  • A total of 430 children were analyzed, revealing unsatisfactory vaccination rates for several key diseases, with only 8.8% fully immunized at diagnosis and inconsistent screening practices for infections like EBV and TB.
  • The findings indicate that despite existing guidelines, immunization and screening for opportunistic infections in pediatric IBD patients are insufficient, highlighting the need for improved healthcare practices.

Unlabelled: The diagnosis and monitoring of Crohn's disease (CD) represents a diagnosis challenge in which imaging plays an important role.

Aim: In the present paper we aim to demonstrate the role of sonoelastography (SE), performed in addition to hydrosonography (HS), in the evaluation of CD in children and to propose a scoring system for the appreciation of disease activity.

Material And Method: All the patients included into the study were diagnosed with CD and had underwent HS and SE as part of the imaging evaluation.

Article Synopsis
  • Inflammatory bowel disease (IBD) includes conditions like ulcerative colitis and Crohn's disease, which are becoming more common but lack clear causes and cures.
  • Recent research highlights that IBD may result from a mix of genetic, environmental, and immune factors, with altered gut microbiota playing a crucial role in its development.
  • The review discusses current and future treatment options aimed at modifying gut microbiota through approaches like diet changes, probiotics, and fecal transplantation, emphasizing the need for more research on effective strategies.

Background: The combination of the severity of pediatric-onset inflammatory bowel disease (IBD) phenotypes and the need for intense medical treatment may increase the risk of malignancy and mortality, but evidence regarding the extent of the problem is scarce. Therefore, the Porto Pediatric IBD working group of ESPGHAN conducted a multinational-based survey of cancer and mortality in pediatric IBD.

Methods: A survey among pediatric gastroenterologists of 20 European countries and Israel on cancer and/or mortality in the pediatric patient population with IBD was undertaken.

Article Synopsis
  • Gastrointestinal (GI) cancers remain a significant health issue, and gut microbiota plays a crucial role in their development, with many studies highlighting this connection.
  • The paper reviews evidence from various studies on the use of probiotics and prebiotics for preventing GI tumors, managing therapy-related toxicity, and addressing post-operative complications.
  • It emphasizes the need for more detailed research on the interactions between gut microbiota and factors like genetics and diet, as well as the necessity for well-designed clinical trials to guide prevention and treatment strategies.

Article Synopsis
  • Numerous studies have examined the causes and potential treatments of inflammatory bowel disease (IBD), highlighting the role of food nutrients alongside genetic, immunologic, environmental factors, and gut microbiota.
  • The impact of nutrition is particularly significant in children, as the disease and its treatments can hinder growth and development, leading to long-term issues.
  • The review emphasizes the importance of enteral nutrition in managing Crohn's disease and provides updates on parenteral nutrition options for pediatric patients with IBD.

Although its pathogenesis remains still unknown, fibrosing colonopathy (FCP) is considered to be the result of prolonged treatment by high doses of pancreatic enzyme preparations, in a small proportion of patients who present with cystic fibrosis (CF). We present the case of a newborn with meconium ileus (treated by conservative measures), in which, at the age of 3 weeks, the features of intestinal obstruction made necessary the removal of 15 cm of the proximal large intestine. Macroscopical and especially microscopical appearances typical for FCP were found, despite the absence of any enzymatic treatment.
