The authors examined quantitative electroencephalographc (QEEG) coherence in 37 depressed elderly patients and performed 2-year follow-up evaluations. All subjects had equivocal cognitive impairment, but none had delirium or dementia. More than 40% (16/37) recovered from depression, and 38% (14/37) remained well for 2 years.

To determine whether tobacco smoke contributes to the pathogenesis of acute otitis media, chinchillas were exposed to mainstream tobacco smoke or sham conditions (cigarettes not lit) in a Walton smoke exposure machine for 20-minute cycles two or three times daily. After 6 to 8 weeks of daily exposure, 12 chinchillas were nasally injected with Streptococcus pneumoniae, and 18 chinchillas were injected into both middle ears with nontypable Haemophilus influenzae. Smoke or sham exposures were continued for 2 to 4 weeks after injection.

Reactive thrombocytosis following cardiopulmonary bypass surgery is generally considered to be a benign condition which is transient and does not require specific therapy. We describe three patients who developed stroke in association with reactive thrombocytosis in the late recovery period following cardiopulmonary bypass surgery. No evidence of embolic or cerebrovascular disease was detected in any patient.

Using quantitative electroencephalographic coherence (a measure of synchronized electrical activity between brain regions) the authors examined heterogeneity in clinical presentation and outcome inpatients with dementia. Patients (N = 114) with mild-to-moderate dementia of the Alzheimer's type (DAT) or multi-infarct dementia (MID) were examined for coherence from the left hemisphere. More than 70% diagnostic accuracy in distinguishing between DAT and MID subjects was achieved using coherence measures alone.

The authors compared parental reports with medical records for 157 children enrolled in a prospective study of chronic otitis media with effusion between 1987 and 1991. Parents completed a questionnaire about the child's past health history, and the research nurse abstracted history information from the clinic's medical record. Previous insertion of a tympanostomy tube (kappa = 0.

Risk factors for otitis media sequelae and chronicity.

Ann Otol Rhinol Laryngol Suppl

May 1994

Otitis media (OM) is a very common childhood illness, with identified risk factors for chronicity and recurrence. Although a few treatment studies have described clinical factors that predict increased morbidity, there is little information on risk factors for middle ear sequelae of OM. These conditions have an estimated annual incidence of 0.

Tympanoplasty and tympanostomy tubes were developed at the same time and have dramatically changed the treatment of chronic middle ear disease. One hundred forty-nine children who had tubes inserted between ages 6 months and 8 years for chronic otitis media with effusion have been prospectively followed up for an average of 4 years. Fourteen percent developed tympanic membrane perforations.

An otitis media with effusion algorithm developed by Paradise et al has become the basis for many studies of otitis media. However, it has been shown to be too ambitious (low specificity) and too optimistic (absence of fluid does not necessarily mean normal ears). We developed a four-point profile to characterize the condition of the middle ear, but it cannot be used when the eardrum is perforated (with a functioning tube or chronic perforation).

Objective: An application of the clinical otitis media profile is proposed for the evaluation of treatments in clinical studies of otitis media.

Methods: Methods include a statistical test of significance and measures of "treatment difference." This article focuses on the method, not any particular study; however, an example is given to illustrate the ideas.

The consequences of severely limiting the T-cell receptor (TCR) repertoire available for the response to intranasal infection with an influenza A virus or with Sendai virus have been analyzed by using H-2k mice (TG8.1) transgenic for a TCR beta-chain gene (V beta 8.1D beta 2J beta 2.

The effect of the Persian Gulf Crisis on Navy families was studied through admission rates of children to a private psychiatric hospital, marital status and diagnostic categories. During that time there was a statistical increase in admission of young boys under age twelve and all children from remarried families. Almost one-half of these patients were diagnosed with Dysthymia whereas this diagnosis was given only once during the time period before or after the crisis.

A right-handed man with an enlarging bony swelling of his dominant hand developed symptoms of median and ulnar nerve compression. He underwent simple excision of the hamate and histology showed the lesion to be a benign chondroblastoma. At 2-year follow-up he was found to be doing a heavy manual job with no functional problems and there were no signs of recurrence of the tumour.

This case-control study investigated factors associated with late initiation (i.e., initiation after the age of 17 years) of smoking among young women.

Objective: To determine the prevalence of electroencephalogram (EEG) abnormalities at different levels of cognitive impairment and to assess the possible diagnostic usefulness of the test.

Design: Combined prospective assessment of subjects receiving EEGs and retrospective chart review of symptoms and medications.

Setting: Academic geriatric psychiatry service.

Quality of life must be assessed after patients have received intensive care. Long-term survival of intensive care patients should be evaluated. TISS (Therapeutic Intervention Scoring System) is an extremely useful tool for estimating the cost of treatment given to an individual patient.

A chart review was performed to determine the resuscitation status of all in-patients in this hospital on a single day. A decision not to resuscitate had been made for 11 (3.7%) of 293 patients.

The effect of prolonged treatment with calcium channel blockers on potassium and magnesium stores is uncertain. We measured lymphocyte and serum magnesium and potassium in 28 patients treated for hypertension for 6 months with nifedipine or captopril. There was no difference in serum or lymphocyte concentrations in the two groups compared to 45 healthy, normotensive controls.

This report documents the follow-up information on 69 hairy cell leukemia (HCL) patients treated with interferon alpha-2b (IFN) as primary treatment from 1983-86. Follow-up through October 1991 shows only 11 patients have died. Forty-one of the 57 patients completing the intended 12 or more months of initial IFN treatment were eventually considered IFN failures.

An otitis media with effusion algorithm developed by Paradise et al and tested by Cantekin et al has become the basis for many studies of otitis media. However, a two-point scale algorithm (otitis media with effusion-no otitis media with effusion) may be too ambitious (ie, low specificity) and too optimistic (ie, absence of fluid does not necessarily mean normal ear). We propose a four-point profile that characterizes the condition of the middle ear, and we report the validation of the profile against findings at myringotomy.

Fifty consecutive clients achieved pregnancy using a standardization modification of the Billings ovulation method (the Creighton Model Natural Family Planning System). Of 50 clients followed, 38 (76%) achieved pregnancy in the first cycle of fertility-focused intercourse, 45 of 50 (90%) achieved pregnancy by the third cycle and 49 of 50 achieved pregnancy by the sixth cycle (98%).

Three trials were conducted to study the effects of a high flow volume (2.3 mL/s) nipple waterer (HFN) versus a low flow volume (.4 mL/s) nipple waterer (LFN) on the productive performance of broiler chickens as measured by average body weight (kilograms), average feed conversion (kilograms:kilogram), and percentage mortality rate.

Background: The 1985 Minnesota Legislature established guidelines for school-based tobacco-use prevention programming and provided financial incentives to school districts to encourage them to adopt a broad range of preventive measures. The Minnesota-Wisconsin Adolescent Tobacco-Use Research Project was funded by the National Cancer Institute in 1986 to evaluate the Minnesota initiative through two parallel studies.

Methods: The Four Group Comparison Study was a prospective study of 48 school "units" which were randomly assigned to one of four conditions in 1987.
