Type 2 Diabetes (T2D) is a condition that is often associated with obesity and defined by reduced sensitivity of PI3K signaling to insulin (insulin resistance), hyperinsulinemia and hyperglycemia. Molecular causes and early signaling events underlying insulin resistance are not well understood. Insulin activation of PI3K signaling causes mTOR dependent induction of PTEN translation, a negative regulator of PI3K signaling.

Aim: To evaluate calcium hydroxylapatite (CaHA) for the treatment of an aging jawline in a routine setting.
Methods: Five investigators enrolled 35 subjects requesting jawline rejuvenation with CaHA.
Background: Age-associated volume loss is now known to play an important role in the structural changes of the aging face. In the lower face, this manifests as drooping of the corners of the mouth and jowl leading to a loss of the oval jawline of youth. Jawline reshaping by replacing volume has therefore become an indispensable component of modern facial rejuvenation.

An expert panel of French practitioners was convened to establish a consensus on the clinical equivalence in efficacy and safety of OnabotulinumtoxinA (900 KDa) and IncobotulinumtoxinA (neurotoxin free from complexing proteins - 150 KDa). The consensus was divided into three sections incorporating a biological, bibliographic and clinical analysis of the two toxins.

Background: Onabotulinumtoxin A (onabotulinum) has been used for 7 years in the treatment of the aging face. A survey was conducted to identify current practices in France.
Objective: To develop consensual practice recommendations for treating the aging upper- and mid-face with onabotulinum.
Injections of soft-tissue fillers have rapidly become accessible and essential. When dealing with facial aging, it is logical to compensate for the loss of volume, but the optimisation of a younger face involves a 3D strategy as well.

Study Design: We performed a cross-sectional random mail survey of physicians.
Population: Surveys were sent to 187 family physicians and 129 pediatricians practicing in North Carolina with 75% and 86% response rates, respectively.
We report the case of an adult male presenting in shock caused by spontaneous rupture of a pathologically normal spleen. One day before his presentation he had received a single subcutaneous injection of enoxaparin. This was the patient's only clearly identifiable risk factor for spontaneous splenic rupture.

This study examines the question of whether chest pain observation units increase the proportion of chest pain patients with an extended evaluation for cardiac ischemia ("rule out myocardial infarction [MI] evaluation"), decrease the number of missed MIs, and decrease costs. This is a multiple site registry study of 8 established chest pain observation units (complying with the American College Emergency Physician's Observation Section's standards) compared with previous studies on chest pain evaluation without the use of observation (5 studies, 12,405 patients). A total of 23,407 of 444,189 emergency department patients (5.

Methods: This was a retrospective, cross-sectional comparison of adult patients considered at relatively low risk for cardiac ischemia and who were evaluated in 1 of 2 settings: a short-term observation service and an inpatient monitored bed. All patients had an ED final diagnosis of "chest pain," "R/O MI," or "unstable angina" during the 7-month study period.

Vomiting of gastric contents is common among multisystem trauma patients and may cause significant morbidity. A study was conducted to examine whether metoclopramide (Reglan), an antiemetic and promotility agent, could decrease vomiting after administration of oral radiographic contrast in stable multisystem trauma patients undergoing computed tomography (CT) of the abdomen ("trauma CT patients"). The charts of 193 patients listed in the Duke Trauma Registry who underwent abdominal CT scanning from January, 1992 until February, 1993 were reviewed.

To determine what patient characteristics are associated with prolonged emergency department (ED) length-of-stay (LOS) for surgical critical care patients, the charts of 169 patients admitted from the ED directly to the operating room (OR) or intensive care unit (ICU) during a 6-week period in 1993 were reviewed. The ED record was reviewed for documentation of factors that might be associated with prolonged ED LOS, such as use of computed tomographic (CT), radiology special procedures, and the number of plain radiographs and consultants. ED LOS was considered to be the time from triage until a decision was made to admit the patient.

To assess the relative significance of ED patients with dizziness and chest pain in emergency medicine practice, the investigators formed a retrospective chart review of 3,864 adults (age > or = 16 years) seen at the University of North Carolina Hospitals' ED during May and June of 1991. Patients were eligible if they had any complaint of dizziness (n = 259), nontraumatic chest pain (n = 168), or both (n = 18).

Methods: We undertook a prospective cohort study with a 6-month follow-up using data gathered in nine primary care practices in two North Carolina counties. Subjects were 144 dizziness patients examined by primary care physicians.
Although much has been written about the importance of collaboration among physicians in practice-based research, the relationship between the clinical investigator and nonphysician professionals in this setting has received less attention. Nurses, physician assistants, and other office support staff can provide valuable insights pertinent to research design and project implementation. Because nonphysician professionals frequently play a large part in implementing a research project, how well the investigator collaborates with them often determines the success or failure of the project.

Twelve standard textile dyes were analyzed using thin layer chromatography and studied comparatively with dyes supposed to be chemically similar, having the same Colour Index number or Registration Code. According to the chromatograms, Disperse Yellow 3, Disperse Orange 1, Disperse Orange 3 and Disperse Orange 13 seemed, at least in the solvent system used, to be pure, while all the other dyes, in the same eluent system, seemed impure. All the standard Textile Dye allergens were similar to the dyes used in the clothing industry.
