Background: Compared to the general population, Veterans Health Administration (VHA) patients have higher rates of mental illness, chronic pain, and substance use disorders (SUD), conditions that increase risk for opioid-related adverse events. VHA developed the Stratification Tool for Opioid Risk Mitigation (STORM) and mandated case reviews by an interdisciplinary team (IDT) for patients identified as very high risk, a process implemented and led by clinical pharmacist practitioners at the Orlando Veterans Affairs Healthcare System (OVAHCS) in 2018.

Objective: To evaluate and describe the implementation and process for IDT reviews of patients identified as very high risk by the STORM clinical decision support tool at OVAHCS.

Thyroid nodules with indeterminate cytology are increasingly subjected to molecular testing. We evaluated the diagnostic performances of Afirma Genomic Sequencing Classifier (GSC) and ThyroSeq v3 in thyroid nodules with high versus low/intermediate suspicion ultrasound classification. In this prospective cohort study, we analyzed all Bethesda III and IV thyroid nodules that underwent fine-needle aspiration biopsies in the University of California Los Angeles Health System from July 2017 to April 2020.

Methemoglobinemia is a blood disorder in which red blood cells contain methemoglobin, a form of hemoglobin that contains iron in its oxidized state, at levels >1%, often leading to a hypoxic state. It can be acquired or congenital and has been associated with nitrate or nitrite exposure. "Poppers" and "Rush" are slang terms given to drugs of the chemical class called amyl nitrate or isobutyl nitrite, which are typically inhaled and can be found over the counter.

Article Synopsis
  • * Conducted at a tertiary referral center, the trial included 102 adults who underwent surgery and recorded their pain levels and medication use over a week.
  • * Results showed the opt-in group had similar pain levels compared to those who received routine prescriptions, suggesting a safe way to reduce opioid consumption post-surgery.

Introduction: The American Association of Colleges of Pharmacy's vision statement is part of a national strategic endeavor to elevate pharmacy's professional profile. Community populations lack awareness of the unique roles and responsibilities of pharmacists. To positively influence the pipeline of future pharmacists, it is necessary to promote pharmacy as a career path.

Context: Primary hyperparathyroidism (PHPT), a leading cause of hypercalcemia and secondary osteoporosis, is underdiagnosed.

Objective: This work aims to establish a foundation for an electronic medical record-based intervention that would prompt serum parathyroid hormone (PTH) assessment in patients with persistent hypercalcemia and identify care gaps in their management.

Methods: A retrospective cohort study was conducted in a tertiary academic health system of outpatients with persistent hypercalcemia, who were categorized as having classic or normohormonal PHPT.

Article Synopsis
  • Molecular testing for thyroid nodules can help determine cancer risk and reduce unnecessary surgeries, but its impact on patients' quality of life (QOL) is not well understood.
  • This study evaluated QOL in patients with benign and suspicious test results, finding that those with benign results maintained stable QOL during surveillance, while those with suspicious results improved post-surgery.
  • Overall, benign test results do not increase anxiety or depression, and surgery can enhance QOL for patients with suspicious results.

Context: Molecular testing to refine the diagnosis of cytologically indeterminate thyroid nodules has become increasingly popular, but data on long-term durability of test results and the rate of delayed operation are limited.

Objective: Determine the delayed rate of surgical resection in indeterminate nodules with benign/negative molecular testing and the risk of false-negative molecular test results.

Design: Prospective follow-up of the Gene Expression Classifier vs Targeted Next-Generation Sequencing in the Management of Indeterminate Thyroid Nodules randomized controlled trial comparing the diagnostic test performance of Afirma Gene Expression Classifier and ThyroSeq v2.

Importance: Approximately 20% of thyroid nodules display indeterminate cytology. Molecular testing can refine the risk of malignancy and reduce the need for diagnostic hemithyroidectomy.

Objective: To compare the diagnostic performance between an RNA test (Afirma genomic sequencing classifier) and DNA-RNA test (ThyroSeq v3 multigene genomic classifier).

Developing effective strategies to prevent or treat coronavirus disease 2019 (COVID-19) requires understanding the natural immune response to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We used an unbiased, genome-wide screening technology to determine the precise peptide sequences in SARS-CoV-2 that are recognized by the memory CD8 T cells of COVID-19 patients. In total, we identified 3-8 epitopes for each of the 6 most prevalent human leukocyte antigen (HLA) types.

Article Synopsis
  • Thyroid surgeons are shifting towards less aggressive management for low-risk thyroid conditions to avoid overdiagnosis, which requires strong communication skills to explain rationale to patients.
  • A study involving 107 adult patients assessed communication skills using the Makoul Communication Assessment Tool, revealing that 23% of patients fell into the low-outlier group for communication effectiveness.
  • The overall communication scores were high, but the study highlighted a particular concern where 29% of patients felt doctors did not encourage them to ask questions, indicating areas for potential improvement.

Context: The increased incidence of thyroid cancer globally over the past several decades is principally attributed to small, indolent papillary thyroid cancers. A possible concomitant increase in thyroid cancer-specific mortality remains debated.

Objective: The changes in thyroid cancer incidence and incidence-based mortality were assessed using a large population-based cohort over an 18-year period.

Parathyroidectomy (PTx) is the only definitive treatment for primary hyperparathyroidism (PHPT), but is commonly underutilized. Most patients are medically observed, whereas approximately 30% of patients are treated operatively. PTx is a low-risk surgical procedure and the most cost-effective treatment option.
