Background: No currently approved treatment for pediatric plaque psoriasis selectively targets interleukin (IL)-23. In adults, guselkumab (a selective IL-23 inhibitor targeting the p19 subunit) demonstrated substantial efficacy with a favorable safety profile in treating moderate-to-severe plaque psoriasis.

Objective: PROTOSTAR (NCT03451851) evaluated the efficacy and safety of guselkumab in pediatric patients with moderate-to-severe plaque psoriasis.

Article Synopsis
  • - Palmoplantar pustulosis (PPP) is a painful chronic skin disorder affecting the palms and soles, with no approved treatments in Europe and the USA, although guselkumab is approved in Japan.
  • - A phase II study involving 50 Caucasian patients showed that administering 100 mg of guselkumab subcutaneously for 24 weeks significantly reduced the severity of PPP, with a median improvement of 59.6% in the PPP severity index.
  • - The study concluded that guselkumab could be a viable treatment option for Caucasian patients suffering from moderate-to-severe PPP, as indicated by substantial improvements in both clinical and quality of life measures.

Background: Suboptimal influenza and pneumococcal vaccination rates have been reported before the COVID-19 pandemics in certain populations at risk for severe infection. The aim of this longitudinal cohort study was to investigate changes in influenza and pneumococcal vaccination rates and patient perceptions in patients with psoriasis (PsO) before and during the pandemic.

Methods: Data on vaccination, patient and disease characteristics, comorbidity, and patient perceptions were collected with questionnaires before and during the pandemic approximately one year later.

Article Synopsis
  • Methotrexate (MTX) is an established long-term treatment for psoriasis vulgaris (PV), and this study aimed to evaluate its real-world effectiveness and safety over time.
  • The study analyzed data from 55 patients over 247 patient-years, showing a 51.2% reduction in psoriasis severity (PASI) within the first six months, stabilizing afterward; side effects were generally mild.
  • Results indicated no serious adverse events and suggested that low-dose MTX is both effective and safe for long-term therapy, though further investigation into higher-dose regimens is necessary for better efficacy data.

Article Synopsis
  • Atopic dermatitis (AD) and psoriasis vulgaris (PV) are two distinct skin conditions that can sometimes lead to a "Flip-Flop" phenomenon, where patients switch from one disease to the other during treatment.
  • Researchers created and validated a diagnostic algorithm in a study of 300 patients to identify these Flip-Flop cases, enhancing it further with machine learning technology.
  • The algorithm showed high accuracy (89.7%) and specific sensitivities for identifying Flip-Flop patients, marking it as the first validated tool for this purpose.

Article Synopsis
  • Recent developments in biologic drugs for psoriasis raise questions about the necessity of inpatient treatment for the condition.
  • A study analyzed data from 59,500 hospital admissions over a decade in German dermatology departments, finding psoriasis was the main diagnosis for 4322 patients (7.3%).
  • Though overall hospitalization days decreased significantly, inpatient admissions for psoriasis increased substantially, indicating a continued need for specialized care despite effective outpatient treatments.

The study of psoriasis has yielded fundamental new insights into immunologic regulation and innovative therapies in a way that few other diseases have. In this review, we summarize the main features of current psoriasis research with emphasis on pathophysiological processes and the milestones in the approval of various biologics and small molecule drugs. Thus, through psoriasis research, we are gaining a better understanding of the interplay between the components of the innate and adaptive immune systems.

Palmoplantar pustulosis (PPP) is a chronic inflammatory skin disease characterised by neutrophilic granulocyte (neutrophil)-filled pustules on the palms and soles. The pathogenesis of PPP is poorly understood. This study conducted an identification of the immune mediators associated with PPP and an exploration of apremilast treatment effects on them.

Objectives: This study evaluated musculoskeletal ultrasound (MSUS) use by dermatologists previously trained on a novel handheld, chip-based ultrasound device (HHUD) to screen for early PsA.

Methods: Twelve dermatologists were recruited to screen psoriasis patients for PsA using the novel HHUD in one major hospital in Bonn (Germany) and six private practices in surrounding regions. Patient screening was based on medical history, clinical examination, and the GEPARD questionnaire paired with an MSUS examination of up to three painful joints.

While suboptimal pneumococcal vaccination rates have been reported in immunosuppressed patients with rheumatic diseases, data for patients with psoriasis (PsO) or atopic dermatitis (AD) are scarce. Pneumococcal vaccination in Germany is recommended in patients with certain comorbidities, immunosuppression, and/or aged 60 years or above. The aim of this multicenter cross-sectional study was to investigate the pneumococcal vaccination rate in patients with PsO compared to patients with AD and to evaluate patient perceptions.

Generalized pustular psoriasis (GPP) is a rare, severe, potentially life-threatening, autoinflammatory, neutrophilic skin disease that may be accompanied by fever and leukocytosis. This paper describes the current state of knowledge on GPP in terms of classification, (differential) diagnosis and prevalence. We present a comparison of the genetics and pathoimmunology of GPP and psoriasis vulgaris with the central mechanisms of autoimmunology and autoinflammation.

There have been multiple systemic drugs approved for the therapy of psoriasis vulgaris and psoriasis arthritis (PsA) in the last decade. However, treatment decisions are difficult to make in women planning a pregnancy and in pregnant and lactating women due to the paucity of data for such cases. The strongest evidence for psoriasis therapy during pregnancy exists for topical corticosteroids.

Background: The most common autoimmune blistering disease, bullous pemphigoid (BP), shows an increased prevalence in psoriatic patients and oncologic patients undergoing immune-checkpoint blockade (ICB). Even though the same autoantigens (BP180/BP230) are detectable, it remains obscure whether clinical or histopathological differences exist between these different groups of BP patients. In this study, we strived to analyze this matter based on own data and previously published reports.

Generalized pustular psoriasis is a severe psoriatic subtype characterized by epidermal neutrophil infiltration. Although variants in IL36RN and MPO have been shown to affect immune cells, a systematic analysis of neutrophils and PBMC subsets and their differential gene expression dependent on MPO genotypes was not performed yet. We assessed the transcriptomes of MPO-deficient patients using single-cell RNA sequencing of PBMCs and RNA sequencing of neutrophils in a stable disease state.
