A small subset of hemodialysis patients exhibits persistently disruptive behavior. When all reasonable attempts to stop such behavior have been exhausted, they may undergo involuntary discharge from their dialysis unit. Such patients typically present repeatedly to the emergency department for urgent inpatient dialysis.

Background: End-stage kidney disease (ESKD) is a significant public health concern disproportionately affecting African Americans (AAs). Type 2 diabetes (T2D) is the leading cause of ESKD in the USA, and efforts to uncover genetic susceptibility to diabetic kidney disease (DKD) have had limited success. A prior genome-wide association study (GWAS) in AAs with T2D-ESKD was expanded with additional AA cases and controls and genotypes imputed to the higher density 1000 Genomes reference panel.

Introduction: Steroid-resistant focal segmental glomerulosclerosis (SR-FSGS) is a common glomerulopathy associated with nephrotic range proteinuria. Treatment goals are reduction in proteinuria, which can delay end-stage renal disease.

Methods: Patients with SR-FSGS were enrolled in a randomized, double-blind placebo-controlled trial of fresolimumab, a monoclonal anti-transforming growth factor-β antibody, at 1 mg/kg or 4 mg/kg for 112 days, followed double-blind for 252 days (NCT01665391).

Background: The presence of population structure in a sample may confound the search for important genetic loci associated with disease. Our four samples in the Family Investigation of Nephropathy and Diabetes (FIND), European Americans, Mexican Americans, African Americans, and American Indians are part of a genome- wide association study in which population structure might be particularly important. We therefore decided to study in detail one component of this, individual genetic ancestry (IGA).

Purpose Of Review: Although the overall mortality rate of patients with end-stage renal disease in the United States continues to decline, cardiac complications remain a leading cause of death in this population. The purpose of this review is to identify principles that can be used to optimize the dialysate concentration of electrolytes in order to reduce the incidence of sudden cardiac deaths (SCDs).
