The 2022 European Society of Cardiology guidelines on cardiovascular assessment and management of patients undergoing non-cardiac surgery are intended for physicians involved in the perioperative care of patients undergoing non-cardiac surgery, in whom heart disease is a potential source of complications. While relevant and useful, the length of the guidelines may limit widespread reading. This article summarises the guidelines, highlighting the aspects most relevant to medical staff preparing patients for surgery.

Background: Mechanical ventilation supports patients with respiratory failure during critical illness. Evidence suggests that excessive tidal volumes (regarded as >8 mL/kg predicted body weight [PBW]) cause lung damage through increased lung stretch and alveolar inflammation. Lung-protective ventilation strategies have been shown to decrease morbidity and mortality, and that all patients should receive tidal volumes between 6 and 8 mls/kg PBW.

In order to gain a better understanding of how to improve the quality of small-molecule single-crystal X-ray diffraction data achievable in a finite time, a study was carried out to investigate the effect of varying the multiplicity, acquisition time, detector binning, maximum resolution and completeness. The results suggest that, unless there are strong arguments for a different strategy, a good routine procedure might be to optimize the conditions necessary to get the best data from single scans, and then choose a multiplicity of observations (MoO) to utilize the available time fully. Different strategies may be required if the crystal is highly absorbing, is larger than the incident beam, is enclosed in a capillary tube or is unusual in some other way.

Article Synopsis
  • Insulinomas are mostly benign tumors of the pancreas that cause low blood sugar due to excess insulin production and typically require surgical removal for treatment.
  • If surgery isn't possible, various medical therapies, including diazoxide and somatostatin analogues, are used, although their effectiveness can vary, and newer targeted therapies like everolimus and sunitinib are being explored.
  • For patients not suitable for surgery, alternative treatments like ethanol ablation or peptide receptor radionuclide therapy (PRRT) may help manage the condition, highlighting the importance of early referral to a specialized neuroendocrine treatment team.

The resonant-scattering contributions to single-crystal X-ray diffraction data enable the absolute structure of crystalline materials to be determined. Crystal structures can be determined even if they contain considerably disordered regions because a correction is available via a discrete Fourier transform of the residual electron density to approximate the X-ray scattering from the disordered region. However, the corrected model cannot normally account for resonant scattering from atoms in the disordered region.
