The authors describe the systematic radiological and serological assessment for a renal cystic mass establishing a preoperative diagnosis. On the basis of this preoperative diagnosis, essential precautions can be taken to avoid the specific complications of this disease and a nephron-sparing procedure can be performed.

A wait-and-see attitude is now possible for benign lesions identified as epidermoid cysts. Pathology examination remains more pertinent for an exhaustive assessment of local extension, but MRI can provide a convincing, if not definitive, distinction between seminomatous and non-seminomatous germ cell tumors.
