Primary aortoenteric fistulae (AEFs) are extremely rare vascular entities, with fewer than 250 cases reported in the world medical literature as of 1996. Incidence is less than 1 per cent, with a mortality ranging from 33 to 85 per cent. Atherosclerosis remains the most common etiology, accounting for more than two-thirds of the cases reported.

The endovascular treatment of peripheral arterial occlusive disease has historically been performed by interventional radiologists and cardiologists. With additional training in endovascular techniques, surgeons become uniquely suited to manage arterial lesions with both endovascular and conventional surgical techniques. Over a 14-month period, 13 patients underwent combination endovascular and open reconstruction on limbs with peripheral arterial occlusive disease.

With trauma being common in this country and over 110,000 recent organ transplants performed, transplant recipients may become trauma victims. At present, only a few older small series of traumatized transplant patients exist. At the University of Arkansas, over the past 40 months, 12 patients with significant trauma were retrospectively identified (seven with kidney and five with combined kidney and pancreas transplants).

In an azotemic patient with suspected renal artery stenosis, undiluted gadopentetate dimeglumine was administered in an aortic bolus to provide contrast at digital subtraction angiography of the aorta and renal arteries. The contrast material (5 mmol/mL) was power injected (rate, 15 mL/sec; total dose, 30 mL). Selective renal arteriography was also performed with 5 mL of undiluted gadopentetate dimeglumine hand injected into each renal artery (total dose, 40 mL).

For combined kidney and pancreas transplant recipients infectious complications remain a major source of morbidity. With as many antibiotic protocols as transplant centers, the exact type and duration for prophylactic wound antibiotics remains undefined. A retrospective review of our series of 40 combined kidney and pancreas transplants was performed using a single 1 g dose of cefazolin preoperatively along with cefazolin bladder and intra-abdominal irrigation.
