Introduction: Spontaneous mesencephalic hemorrhages are very unusual, specially those located in the tectal region. Hypertension is a less important factor that in other classical locations. Other etiologies reported are arteriovenous malformations, coagulation disorder and trauma.

Pleural involvement is unusual and only rarely is it found as an isolated sign. We describe 2 cases of empyema due to actinomycosis; both cases responded well to surgical drainage and antibiotics.
