The transmeatal extension of the retrosigmoid approach is an important procedure used in the treatment of various pathologies affecting the posterior fossa, petroclival region, and jugular foramen. Introduction: Clinoidal meningiomas and paraclinoid aneurysms are individually uncommon, with the coexistence of mirror paraclinoid aneurysms and clinoidal meningiomas presenting an even rarer scenario. While the association between meningiomas and aneurysms is documented, the simultaneous presence of mirror lesions for both pathologies is not reported in the literature.

Clinical Presentation: We report a 62-year-old female with a three-month history of moderate bifrontal headaches.
