Frailty and emergency surgery: identification and evidence-based care for vulnerable older adults.

Frailty is a multidimensional state related to accumulation of age- and disease-related deficits across multiple domains. Older people represent the fastest growing segment of the peri-operative population, and 25-50% of older surgical patients live with frailty. When frailty is present before surgery, adjusted rates of morbidity and mortality increase at least two-fold; the odds of delirium and loss of independence are increased more than four- and five-fold, respectively.

Initial Experience With High-Risk Patients Excluded From Clinical Trials: Safety of Short-Term Anticoagulation After Left Atrial Appendage Closure Device.

Background: The implantation of left atrial appendage closure device (WATCHMAN, Boston Scientific, Natick, MA) is an alternative option to oral anticoagulation (OAC) for stroke prevention in atrial fibrillation. Patients require short-term OAC after implantation to avoid device thrombosis. The 2 clinical trials that assessed this device excluded patients thought not to be candidates for OAC.

Longer-baseline telescopes using quantum repeaters.

We present an approach to building interferometric telescopes using ideas of quantum information. Current optical interferometers have limited baseline lengths, and thus limited resolution, because of noise and loss of signal due to the transmission of photons between the telescopes. The technology of quantum repeaters has the potential to eliminate this limit, allowing in principle interferometers with arbitrarily long baselines.

Kinematics of heelstrike during walking and carrying: implications for slip resistance testing.

Slip resistance measurements of shoes and floors are used to evaluate the potential for slip and fall injuries. These measurements are believed to have increased validity when they more closely reflect actual heelstrike biomechanics during locomotion. The purpose of this study was to describe heelstrike kinematics during load carrying to provide data towards improved slip resistance testing.
