Objectives: The study institute was challenged with an outbreak of different vancomycin-resistant Enterococcus faecium (VREfm), including vanA- and/or vanB-containing isolates. Remarkably, screening overnight enriched specimens using a vanA and vanB real-time polymerase chain reaction (PCR) gave positive results for vanB with very low cycle threshold values, whereas VREfm-specific enrichment cultures remained negative. This paper describes the analysis of the diagnostic results leading to adaptation of the diagnostic algorithm.

By longitudinally following a large cohort of intercontinental travellers, this study highlights the importance of considering multiple risk factors to comprehend post-infectious irritable bowel syndrome (PI-IBS). Stomach cramps, antibiotic use and nausea during travel were amongst the variables that predicted PI-IBS development following an episode of traveller’s diarrhoea.

Background: The appropriate duration of postoperative antibiotics for complex appendicitis is unclear. The increasing global threat of antimicrobial resistance warrants restrictive antibiotic use, which could also reduce side-effects, length of hospital stay, and costs.

Methods: In this pragmatic, open-label, non-inferiority trial in 15 hospitals in the Netherlands, patients with complex appendicitis (aged ≥8 years) were randomly assigned (1:1) to receive 2 days or 5 days of intravenous antibiotics after appendicectomy.
