Registered nurse (RN) job satisfaction is a major predictor of intent to stay and job turnover, serious concerns to health care leaders. Predictors of job satisfaction include autonomy, control over daily practice, nurse-physician collaboration, transformational leadership, group cohesion, job stress, structural empowerment, and psychological empowerment. In the model of psychological empowerment, stress resiliency is the product of persons' interpretive styles and influences psychological empowerment.

This exemplar highlights how a rural academic nursing practice has been effective in facilitating the integration of the tripartite academic role of education, research, and service for one university faculty member. The integration of students into the practice is described. The experience provided them with critical insights into rural academic practice during their delivery of prenatal care services to a vulnerable population of women in rural Appalachia.

Home visiting may be a promising strategy to improve pregnancy outcomes, and home visiting by lay workers may be more accepted by pregnant women. Lay workers may impact on social and environmental risk factors as well as on health care utilization. As with any primary prevention strategy, these programs may be more successful if implemented with responsibility shared between the health care system and the community.
