Article Synopsis
  • Precise identification of nerve fascicles is crucial for certain surgeries on peripheral nerves, with bipolar nerve stimulation preferred, but many still use handheld monopolar stimulators.
  • The study presents a new technique for monopolar stimulation, where both the anode and cathode are placed on the same nerve fascicle to enhance specificity.
  • Two surgical cases demonstrated that this modified method allowed for better selection of nerve fascicles, resulting in successful outcomes without unexpected complications.

Background: Transfer of the fascicle carrying the flexor carpi ulnaris (FCU) branch of the ulnar nerve (UN) to the biceps/brachialis muscle branch of the musculocutaneous nerve (Oberlin's procedure), is a mainstay technique for elbow flexion restoration in patients with upper brachial plexus injury. Despite its widespread use, there are few studies regarding the anatomic location of the donor fascicle for Oberlin's procedure. Our report aims to analyze the anatomical variability of this fascicle within the UN, while obtaining quantifiable, objective data with intraoperative neuromonitoring (IONM) for donor fascicle selection.

Article Synopsis
  • - Regenerative Peripheral Nerve Interfaces (RPNIs) utilize nerve grafts to enhance electrical signaling in peripheral nerves, and this study reviewed existing research on RPNIs using animal models from 1970 to 2023.
  • - A total of 104 articles on RPNIs were analyzed, with 35 specifically focusing on animal studies, predominantly using mice (91%) and a smaller number using macaques (9%).
  • - Key findings showed significant axonal regeneration and improved blood vessel formation, with RPNIs effectively able to detect and amplify nerve signals, indicating promising long-term performance that can guide future research.

Article Synopsis
  • A study was conducted on early-stage breast cancer patients without diabetes who received dexamethasone alongside taxane chemotherapy from August 2017 to December 2019.
  • The research found that 67% of these patients experienced steroid-induced hyperglycemia (SIH), with significant glycemic fluctuations in those reaching glucose levels above 200 mg/dL.
  • Non-Hispanic White patients had a notably higher risk for developing SIH, and over 90% of patients saw their hyperglycemia resolve after treatment, with only a small number remaining hyperglycemic post-treatment.

Lymphedema is a chronic disease with a high incidence in our society. In this paper, we present a review with the latest advances in imaging techniques and surgical reconstructive treatment of lymphedema (lymphovenous anastomosis, vascularized lymph node transfer, and prophylactic lymphedema surgery). In addition, a protocol is established based on a multidisciplinary team (composed of physiatrists, plastic surgeons, radiologists and nuclear medicine radiologists) to optimize the treatment of these patients.

Immunodeficient mouse models with human skin xenografts have been developed in the past decades to study different conditions of the skin. Features such as follow-up period and size of the graft are of different relevance depending on the purpose of an investigation. The aim of this study is to analyze the different mouse models grafted with human skin.

Background: One of the challenges in head and neck reconstruction is to have an adequate understanding of the three-dimensionalities of the defects created after resections due to the high variability of clinical scenarios. Consequently, it is essential to design the flap to match the requirements of the defect in order to facilitate the insetting and to achieve a successful outcome. The anterolateral thigh flap (ALT) is a robust and versatile flap commonly used in head and neck reconstruction.

Background: Prior studies demonstrate that social media are used by plastic surgeons to educate and engage. The hashtag #PlasticSurgery has been studied previously and is embraced by American plastic surgeons and journals; however, no studies have examined its use or adoption across Europe.

Methods: A retrospective analysis of 800 tweets containing the words "plastic surgery" or the hashtag #PlasticSurgery in four of the most spoken European languages worldwide excluding English (Spanish, #CirugiaPlastica; French, #ChirurgiePlastique; Portuguese, #CirurgiaPlastica; and German, #PlastischeChirurgie) was performed.

The growth hormone is involved in skin homeostasis and wound healing. We hypothesize whether it is possible to improve pressure ulcer (PU) healing by locally applying the recombinant human growth hormone (rhGH) in a human skin mouse model. Non-obese diabetic/severe combined immunodeficient mice ( = 10) were engrafted with a full-thickness human skin graft.

Despite advances in regenerative medicine and tissue engineering, human skin substitutes remain a clear goal to achieve. Autografts remain the principal clinical option. The long-term changes in dermis, as well as its response after injuries, are not well known.

Background: Radical tumor ablation in the periauricular area often results in extensive soft tissue defects, including facial nerve sacrifice, bone and/or dura defects. Reconstruction of these defects should aim at restoring facial reanimation, wound closure, and facial and neck contours. We present our experience using free anterolateral thigh flap (ALT) in combination with masseter nerve to facial nerve transfer in managing complex defects in the periauricular area.

Aim: Sacral nerve stimulation (SNS) lead implantation is a straightforward procedure for individuals with intact spinal vertebrae. When sacral anomalies are present, however, the anatomical and radiological reference points used for the accurate placement of the electrode may be absent or difficult to identify.

Method: We describe an innovative surgical procedure of percutaneous nerve evaluation for SNS in a patient with faecal incontinence secondary to a congenital imperforate anus and partial sacral agenesis using a surgical imaging platform (O-arm system) under neurophysiological control.

Introduction: Pressure ulcers are a prevalent health problem in today's society. The shortage of suitable animal models limits our understanding and our ability to develop new therapies. This study aims to report on the development of a novel and reproducible human skin pressure ulcer model in mice.

Primary lower-limb lymphoedema is a chronic, progressive and debilitating condition with a difficult management, especially in advanced cases (elephantiasis). Recently, autologous lymph node transplantation (ALNT) appears to be a promising treatment for extremity lymphoedema. A case of a double ALNT for an advanced primary lower-limb lymphoedema is here reported: a contralateral inguinal lymph node flap was transferred to the knee and, in a second surgery, a thoracic lymph node flap was transplanted to the inguinal region.

The benefits of cardiac rehabilitation for patients with heart failure are well documented: greater capacity during physical effort and improved quality of life, a reduction in comorbidities and in the number and duration of hospitalisations, etc. Physiotherapy is one of the tools of this specific and multidisciplinary care which is governed by protocols and which can be offered on an outpatient basis or during hospitalisation.
