Introduction: The Sustainable Development Goals of the United Nations include a commitment to "leave no one behind" as a universal goal. To achieve this in geriatric oncology (GO) worldwide, it is important to understand the current state of GO at an international level. The International Society of Geriatric Oncology (SIOG) has several National Representatives (NRs) who act as SIOG's delegates in their respective countries.

Article Synopsis
  • PARP inhibitors have shown significant benefits in prolonging progression-free survival (PFS) for patients with relapsed, platinum-sensitive epithelial ovarian cancer (OC).
  • This study analyzed the effects of the most recent platinum-based chemotherapy on the response to PARP inhibitors in 96 advanced OC patients.
  • Results indicated that those who received pegylated liposomal doxorubicin-oxaliplatin (PLD-Ox) prior to PARP inhibitors experienced better overall survival (OS) and PFS, especially in patients with BRCA mutations.

Endocrine therapy represents the cornerstone of treatment in hormone receptor-positive (HR+), HER2-negative metastatic breast cancer (mBC). The natural course of this disease is marked by endocrine resistance, mainly due to Estrogen Receptor 1 (ESR1) acquired mutations. The aim of this study is to evaluate the concordance between ESR1 status in metastatic tumor specimens and matched circulating tumor DNA (ctDNA).

  • Previous studies showed that steady-state Z-endoxifen levels (ENDOss) are linked to better cancer outcomes for women on tamoxifen.
  • A study involving 203 Caucasian women evaluated the reliability of two methods for measuring CYP2D6 activity: genetic testing and a urinary dextromethorphan test.
  • It was found that the urinary log(DM/DX) test was a better predictor of ENDOss than genetic testing, highlighting the importance of considering this approach in breast cancer treatment.

  • A multicentre trial named ICOS-ONE found that 27% of patients showed increased cardiac troponin levels after receiving anthracycline chemotherapy, prompting a post hoc analysis to explore long-term cardiotoxicity and monitor cardiovascular biomarkers.
  • The study involved patients with no prior cardiovascular issues, showing no significant differences in biomarker levels between those receiving preventative treatment and those treated upon experiencing elevated levels.
  • Over 36 months, there were no new cases of left ventricular dysfunction or elevated troponin; BNP levels remained stable, while PTX3 peaked shortly after treatment and then returned to baseline levels, indicating stable heart function overall.

Background: Troponin changes over time have been suggested to allow for an early diagnosis of cardiac injury ensuing cancer chemotherapy; cancer patients with troponin elevation may benefit of therapy with enalapril. It is unknown whether a preventive treatment with enalapril may further increase the benefit.

Methods: The International CardioOncology Society-one trial (ICOS-ONE) was a controlled, open-label trial conducted in 21 Italian hospitals.

Purpose: A multidimensional prognostic index (MPI) based on a comprehensive geriatric assessment (CGA) has been developed and validated in independent cohorts of older patients demonstrating good accuracy in predicting one-year mortality. The aim of this study was to develop a cancer-specific modified MPI (Onco-MPI) for mortality prediction in older cancer patients.

Methods: We enrolled 658 new cancer subjects ≥70 years (mean age 77.

The introduction of trastuzumab into routine clinical practice has had a dramatic effect on the outlook for patients with HER2-positive breast cancer. Nevertheless, answers to some long unresolved questions about the optimal use of trastuzumab (such as its role in small tumors or low-risk disease, cardiac safety in the elderly, and treatment duration) have emerged only relatively recently. Moreover, with the availability of new highly effective HER2-directed therapies, including pertuzumab and trastuzumab-emtansine (T-DM1), the treatment algorithm for HER2-positive breast cancer continues to evolve.

Objective: Some parameters of the Comprehensive Geriatric Assessment (CGA) are predictive of chemotherapy toxicity. The Vulnerable Elders Survey-13 (VES-13) is a short instrument that has been tested as a means of identifying patients who need a full CGA, but its ability to predict chemotherapy toxicity is still unclear. We performed a pooled analysis of four published clinical trials studying VES-13 as a means of diagnosing vulnerability, in order to evaluate its accuracy in predicting the risk of grade 3/4 toxicity in older patients undergoing chemotherapy.

Over the past years, although no increase in the cure rate for advanced epithelial ovarian cancer patients has been achieved, a slow prolongation in patients survival has been observed, thanks to the introduction of effective second line or salvage therapies. Attempts to disease chronicization seem therefore of value in this setting. A major effort has been pursued to establish the role of maintenance therapies for epithelial ovarian cancer patients.

Article Synopsis
  • The study focuses on evaluating the effectiveness of the Multidimensional Prognostic Index (MPI) in elderly cancer patients (ECP) with advanced stages of disease, highlighting the need for Comprehensive Geriatric Assessment (CGA) before treatment decisions.
  • A total of 160 patients aged 70 and above with inoperable or metastatic solid cancer were assessed, revealing that measures like MPI, CIRS-SI, and others were significant indicators of mortality within 6 and 12 months.
  • The findings suggest that MPI remains a valuable prognostic tool even for older patients with serious cancer, and combined assessments with CIRS-SI and GDS can enhance its predictive capabilities.
