Count Neipperg (1775-1829), the morganatic husband of Maria Luigia of Habsburg, Napoleon's former wife, presented with typical heart failure symptoms and died of bilateral bronchopneumonia. Neipperg's case is an example of the conflict in the medical field, which led to the birth of modern evidence-based medicine (EBM), and although Neipperg died almost 200 years ago, his case presents the same critical issues that more complex geriatric patients face today.   • This study examines the impact of starting PCSK9 inhibitors (PCSK9i) at the time of hospitalization for acute coronary syndrome (ACS) on lipid levels and cardiovascular (CV) events in a real-world setting.
  • It analyzed data from 771 ACS patients, revealing that a significant 68.3% achieved the target LDL-C of < 55 mg/dL within a median of 37 days after hospitalization.
  • Results showed that lower LDL-C levels were associated with reduced risks of major cardiovascular events, suggesting that early and aggressive lipid management with PCSK9i is both safe and beneficial for ACS patients.

The paradigm of the framing of Parkinson's disease (PD) has undergone significant revision in recent years, making this neurodegenerative disease a multi-behavioral disorder rather than a purely motor disease. PD affects not only the "classic" substantia nigra at the subthalamic nuclei level but also the nerve nuclei, which are responsible for sleep regulation. Vitamin D is a group of lipophilic hormones with pleiotropic actions. It has been traditionally related to bone metabolism, although several studies in the last decade have suggested its role in sarcopenia, cardiovascular and neurological diseases, insulin-resistance and diabetes, malignancies, and autoimmune diseases and infections. In the pandemic era, by considering the response of the different branches of the immune system to SARS-CoV-2 infection, our aims are both to analyse, among the pleiotropic effects of vitamin D, how its strong multimodal modulatory effect on the immune system is able to affect the pathophysiology of COVID-19 disease and to emphasise a possible relationship between the well-known circannual fluctuations in blood levels of this hormone and the epidemiological trend of this infection, particularly in the elderly population.
