Proliferative Glomerulonephritis with Monoclonal IgG Deposits (PGNMID) is a glomerular disease characterized by membranoproliferative and mesangioproliferative lesions, with granular capillary wall monoclonal IgG positivity and immunoglobulin light chain restriction. Most commonly a disease of older adults, we present the case of an 18-year-old patient who developed de novo PGNMID in a kidney allograft three years after kidney transplantation. There was minimal proteinuria and no serum paraproteinemia was detected, so the patient was managed conservatively.

The number of patients waiting for kidney transplants from deceased organ donors continues to increase. In this context, non-transplantation of acceptable kidneys is especially regrettable. Here, we review successful transplantation of deceased donor kidneys with anatomic abnormalities, intrinsic kidney diseases, and other ostensibly problematic conditions.

Article Synopsis
  • - Regulatory CD8 T cells (CD8 Treg) usually help control harmful CD4 T cells, but in autoimmune diseases, they often fail, partly due to inhibitory receptors that limit their activation.
  • - A new bispecific antibody called MTX-101 targets these inhibitory receptors and CD8 T cells to enhance the ability of CD8 Treg to eliminate pathogenic CD4 T cells in various experimental models.
  • - In studies, MTX-101 improved CD8 Treg activity, reduced harmful CD4 T cell expansion, and protected tissues from damage without triggering excessive inflammation, showing promise for treating autoimmune disorders.

  • Patterns of failure and salvage treatment options for anal squamous cell carcinoma (ASCC) post-definitive IMRT were investigated, revealing limited data on patient outcomes after recurrence.
  • A study of 375 ASCC patients showed significant locoregional failure (12%) and distant failure (13%) rates, with 73% progression-free survival and 80% overall survival at six years, emphasizing poorer outcomes for those with locoregional failure.
  • Salvage therapies improved survival chances significantly, with 30% of patients alive after ten years, contrasting sharply with those receiving only chemotherapy or supportive care.

Stereotactic ablative radiotherapy (SABR) commonly is used for small liver metastases. Modern conformal radiotherapy techniques, including 3-dimensional conformal radiotherapy and intensity-modulated radiation therapy, enable the safe delivery of SABR to small liver volumes. For larger tumors, the safe delivery of SABR can be challenging due to a more limited volume of healthy normal liver parenchyma and the proximity of the tumor to radiosensitive organs, such as the stomach, duodenum, and large intestine.

  • Adjuvant mFOLFIRINOX (mFFX) is a standard treatment for patients with resected pancreatic ductal adenocarcinoma (PDAC), but there is limited information on its effectiveness outside of clinical trials.
  • A study of 147 patients showed a median recurrence-free survival (RFS) of 26 months, with some patients over 70 years experiencing a median overall survival (OS) of 51 months.
  • Starting mFFX treatment within 8 weeks of surgery was linked to better survival outcomes, while certain genetic factors like KRAS mutations suggested poorer RFS and OS.

Article Synopsis
  • Total mesorectal excision with intersphincteric resection and handsewn coloanal anastomosis (ISR-CAA) is considered safe for patients with distal rectal cancer, but the outcomes for those not qualifying for a watch-and-wait strategy have yet to be studied.
  • A retrospective analysis compared ISR-CAA with abdominoperineal resection (APR) in patients who received neoadjuvant therapy, showing similar tumor characteristics but differing local recurrence rates.
  • Results revealed a lower 5-year local recurrence-free survival rate for ISR-CAA (79%) compared to APR (93%), while disease-free survival rates were similar for both groups.

Background: Potential differences in organ preservation between total neoadjuvant therapy (TNT) regimens integrating long-course chemoradiotherapy (LCCRT) and short-course radiotherapy (SCRT) in rectal cancer remain undefined.

Patients And Methods: This natural experiment arose from a policy change in response to the COVID-19 pandemic during which our institution switched from uniformly treating patients with LCCRT to mandating that all patients be treated with SCRT. Our study includes 323 locally advanced rectal adenocarcinoma patients treated with LCCRT-based or SCRT-based TNT from January 2018 to January 2021.

The use of radiation therapy (RT) for pancreatic cancer continues to be controversial, despite recent technical advances. Improvements in systemic control have created an evolving role for RT and the need for improved local tumor control, but currently, no standardized approach exists. Advances in stereotactic body RT, motion management, real-time image guidance, and adaptive therapy have renewed hopes of improved outcomes in this devastating disease with one of the lowest survival rates.

Introduction: Immune checkpoint inhibitor (ICI) combinations extend overall survival (OS) while anti-PD-1/L1 monotherapy is non-inferior to sorafenib in treatment-naïve, patients with advanced hepatocellular carcinoma (HCC). Clinicogenomic features are posited to influence patient outcomes.

Methods: The primary objective of this retrospective study was to define the clinical, pathologic, and genomic factors associated with outcomes to ICI therapy in patients with HCC.

Background: Neoadjuvant therapy leads to a clinical complete response in a considerable proportion of patients with locally advanced rectal cancer, allowing for possible nonoperative management. The presence of mucin on magnetic resonance imaging (MRI) after neoadjuvant therapy leads to uncertainty about residual disease and appropriateness of a watch-and-wait strategy in patients with no evidence of disease on proctoscopy (endoscopic clinical complete response).

Methods: MRI reports for locally advanced rectal cancer patients seen between July 2016 and January 2020 at Memorial Sloan Kettering Cancer Center were queried for presence of mucin in the tumor bed on MRI following neoadjuvant therapy.

School-based mindfulness trainings (SBMT) are a contemporary approach for intervening to promote students' social and emotional skills and well-being. Despite evidence from the larger field of evidence-based social and emotional learning programs demonstrating the importance of high-quality implementation, few studies have investigated factors impacting the implementation of SBMTs, particularly teacher-level influences. The present study addressed this issue by investigating whether teachers' stress, trust in their fellow teachers and principal, and expectations about the program at baseline predicted the quality of their implementation of a SBMT for students.

Background: Palmar-plantar erythrodysesthesia (PPE) is a slowly developing cutaneous reaction commonly experienced by patients treated with fluoropyrimidines. While erythrodysesthesia normally presents in a palmar-plantar distribution, it can also present with genital involvement, but this presentation is likely underreported and incorrectly attributed to an acute reaction from radiation therapy. This article aims to define erythrodysesthesia of the penis and scrotum as a rare but significant side effect of capecitabine.

HLA matching in solid organ transplant is performed with the aim of assessing immunologic compatibility in order to avoid hyperacute rejection and assess the risk of future rejection events. Molecular mismatch algorithms are intended to improve granularity in histocompatibility assessment and risk stratification. PIRCHE-II uses HLA genotyping to predict indirectly presented mismatched donor HLA peptides, though most clinical validation studies rely on imputing high resolution (HR) genotypes from low resolution (LR) typing data.

(1) Osteoarthritis is a degenerative joint disease that is commonly diagnosed in the aging population. Interestingly, the lower extremity joints have a higher published incidence of osteoarthritis than the upper extremity joints. Although much is known about the disease process, it remains unclear why some joints are more affected than others.

Superparamagnetic iron oxide nanoparticles (SPION) have attracted great attention not only for therapeutic applications but also as an alternative magnetic resonance imaging (MRI) contrast agent that helps visualize liver tumors during MRI-guided stereotactic body radiotherapy (SBRT). SPION can provide functional imaging of liver parenchyma based upon its uptake by the hepatic resident macrophages or Kupffer cells with a relative enhancement of malignant tumors that lack Kupffer cells. However, the radiomodulating properties of SPION on liver macrophages are not known.
