Influence of dietary methionine concentration on growth and nitrogen balance in weanling Quarter Horses.

Twenty-four Quarter Horse weanlings (120 ± 10 d) were blocked by age into 4 groups (n=6) for a 56-d trial to evaluate the influence of dietary Met concentration on growth and N balance. Weanling horses were housed by block and individually fed concentrates twice daily at 1.75% BW (as-fed basis).

Assessing safe and independent living in vulnerable older adults: perspectives of professionals who conduct home assessments.

Objectives: The objectives of this study were to describe social services and health professionals' perceptions of vulnerability among older adults living in the community and to elicit how these professionals screen vulnerability in community and in-home settings.

Methods: Focus group sessions were conducted and analyzed using standardized methods of qualitative analysis. Participants included social services and health professionals (n = 45) who routinely encounter vulnerable older adults.

This column uses the tools of normative ethics-analysis and argument-to provide a reasoned account of and to identify ethically justified responses by the psychiatrist to psychiatric inpatients' refusal of medical or surgical diagnostic work-up. There are three relevant ethical considerations when psychiatric inpatients refuse medical or surgical diagnostic tests: balancing autonomy with beneficence, surrogate decision making and confidentiality, and managing strong feelings. Assisted decision making and assent are key management strategies for promoting patients' autonomy and for protecting against adverse consequences of decision making.

Prevalence and treatment of narcissistic personality disorder in the community: a systematic review.

Background: Few studies have examined the prevalence and treatment of narcissistic personality disorder (NPD).

Method: We systematically reviewed studies of NPD that used suitable diagnostic methods in adult nonclinical samples and evaluated their strengths and weaknesses. Searches were conducted of MEDLINE (using both MeSH category and free-word search terms), PsycINFO, and PsycLIT for articles in English from January 1980 to August 2008 using the terms Narcissis* and prevalence, of unpublished work identified via contacts with experts in the field, of books on personality disorders, and of reference lists from relevant articles and books.

Toolbox for evaluating residents as teachers.

Objective: The authors review existing assessment tools related to evaluating residents' teaching skills and teaching effectiveness.

Methods: PubMed and PsycInfo databases were searched using combinations of keywords including "residents," "residents as teachers," "teaching skills," and "assessments" or "rating scales."

Results: Eleven evaluation tools that utilized self-reports, learner evaluations, or observed structured teaching evaluations were found.

Evaluating the quality of learning-team processes in medical education: development and validation of a new measure.

Background: Measurement of the quality of team processes in medical education, particularly in classroom-based teaching settings, has been limited by a lack of measurement instruments. Therefore, the purpose of this study was to develop and test an instrument to measure the quality of team interactions.

Method: The authors created 30 items and reduced these to 18 items using factor analysis.

Patient aggression perceived by community support workers.

Objective: Aggression by patients is a known risk factor for hospital workers. Within New Zealand, the bulk of ongoing care for physical and mental disabilities and health issues is not hospital based, but contracted to various non-governmental agencies. The rate of client aggression towards care workers from these organizations, to our knowledge, has not been assessed.

Seizures associated with levofloxacin: case presentation and literature review.

Purpose: We present a case of a patient who developed seizures shortly after initiating treatment with levofloxacin and to discuss the potential drug-drug interactions related to the inhibition of cytochrome P450 (CYP) 1A2 in this case, as well as in other cases, of levofloxacin-induced seizures.

Methods: Several biomedical databases were searched including MEDLINE, Cochrane and Ovid. The main search terms utilized were case report and levofloxacin.

The 12-month prevalence of patient-initiated aggression against psychiatrists: a New Zealand national survey.

Objectives: We surveyed the prevalence of various types of threats or assaults by patients against psychiatrists and determined the impact of the most distressing event.

Method: An anonymous questionnaire was mailed to all 308 vocationally registered psychiatrists practicing in New Zealand.

When should psychiatrists seek criminal prosecution of assaultive psychiatric inpatients?

This Open Forum commentary reviews the ethical considerations relevant to the question of prosecuting assaultive psychiatric patients, with particular attention to the significance that should be attached to the arguments generated by those considerations. A comprehensive literature search was conducted incorporating the terms "assaultive patients," "ethics," "psychiatric inpatients," and "law." The literature of professional medical ethics was applied to identify relevant domains of ethical argument.

The ethics of psychiatric education.

Evidence-based medicine and the professional virtues of integrity, compassion, self-effacement, and self-sacrifice constitute the cornerstones of an ethics of psychiatric education. As informed by the pioneering work of John Gregory, psychiatric educators must promote evidence-based and ethically justified behaviors in learners and practicing physicians through example and by formal teaching. These processes together will enable patients to trust the competence of psychiatrists and that psychiatrists will serve the interests of patients first.

Prepublication review of medical ethics research: cause for concern.

Purpose: Although medical ethics research is frequently published in medical journals, little is known about the prepublication review such articles receive. The authors sought to describe the prepublication review of medical ethics research at major medical journals and medical ethics journals. They also sought to compare the ethics backgrounds of editors at both journal types and editors' attitudes concerning the training necessary for reviewers of ethics research.
