When it rains, it pours: Early treatment with tecovirimat of cardiac complications associated with monkeypox infection in a person with HIV and previously undiagnosed Lyme disease. A case report.

Cardiac involvement, such as myocarditis and pericarditis, can be a severe complication of monkeypox virus (mpox) infection and could be related to other co-infections with cardiac involvement. Tecovirimat is an antiviral specifically designed to inhibit smallpox infection diffusion and approved by the FDA for other Orthopoxvirus infections; its efficacy in mpox-infected patients is not well established. We present the case of a cardiac complication during mpox infection in a previously undiagnosed Lyme disease in a 42-year-old man living with HIV.

We investigate the possibility of jamming the opening of the Receptor Binding Domain (RBD) of the spike protein of SARS-CoV-2 with small compounds. Through in silico screening, we identified two potential candidates that would lock the Receptor Binding Domain (RBD) in a closed configuration, preventing the virus from infecting the host cells.

In the United States, most floodplain relocation (or buyout) programs focus on moving homeowners, then deal separately with what happens with the land afterward. These programs typically divide processes for relocation planning, engagement, funding, and implementation from those related to post-buyout land management and restoration. The structural and operational conditions that lead to this separation of roles and responsibilities miss out on opportunities to create more synergistic socio-ecological strategies that may produce healthier outcomes for both people and the environment.

Paratesticular fibrous pseudotumor with histological features of IgG4-related disease: two case reports and review of the literature.

Background: Paratesticular fibrous pseudotumor (PFP) is a rare intrascrotal benign fibrous mass of uncertain aetiology, usually arising between testicular tunica layers and is supposed to be related to inflammatory reactive conditions. Because of morphological similarities to IgG4-related sclerosing fibro-inflammatory lesions, some authors recently postulated that PFP might belong to the IgG4-related disease (IgG4-RD) family. Considering the rarity of this lesion, only few cases have been reported in literature about the correlation between IgG4-RD and PFP.

On the role of polydispersity on the phase diagram of colloidal rods.

The rich and complex phase diagram typical of anisotropic biological or synthetic nanoparticles, has brought a great deal of interest over the equilibrium phase behaviour of non-spherical colloids. Amongst the class of anisotropic nanoparticles, hard spherocylindrical colloids have been, over the years, extensively studied because of their optical properties, for their rich phase diagrams, and their important industrial applications, as model particles for biological systems (viruses), or for example as potential drug carriers having the ability of surviving the attacks of the immune systems. As real anisotropic nanoparticles are often polydisperse in size and/or in shape, unveiling the effect of such a perturbation over their equilibrium phase diagram is of paramount importance.

Overview of general surgery medical residency programs and prerequisite program in basic surgical area in Brazil: Historical review and update.

Objective: to describe vacancy regulation process by adding, describing the panorama of the General Surgery Residency Program (PRMCG) and the Basic Surgical Prerequisites Program (PRACB).

Method: descriptive, quali-quantitative, cross sectional study conducted from document analysis from National Commission of Medical Residency (CNRM).

Results: in 2018, after evaluation of the General Surgery Services for adequacy of the number of vacancies (DS), the PRACB was instituted as a modality of access to surgical specialties until definitive change in the time of the formation of the general surgeon for three years, in 2022.

Unveiling adsorption generality in polymeric macromolecules.

Unveiling a general law for adsorption within macromolecules sets an important step forward in the design of nanomaterials with tunable and controllable properties. Reaching such a generalised control would have an important outcome in a plethora of possible fields, from biomedical applications up to materials science. In fact, the definition of classes regrouping adsorbing macromolecules with different geometrical or chemical properties would enormously simplify the design of controllable adsorbing materials, especially when geometrical or chemical constraints are set by the particular application.

Robotic-assisted, laparoscopic, and open radical cystectomy: surgical data of 1400 patients from The Italian Radical Cystectomy Registry on intraoperative outcomes.

Introduction: The Italian Radical Cystectomy Registry (Registro Italiano Cistectomie - RIC) aimed to analyse outcomes of a multicenter series of patients treated with radical cystectomy (RC) for bladder cancer.

Material And Methods: An observational, prospective, multicenter, cohort study was performed to collect data from RC and urinary diversion via open (ORC), laparoscopic (LRC), or robotic-assisted (RARC) techniques performed in 28 Italian Urological Departments. The enrolment was planned from January 2017 to June 2020 (goal: 1000 patients), with a total of 1425 patients included.

Case Report: Intravenous Pentamidine Rescue Treatment for Active Chronic Visceral Leishmaniasis in an HIV-1 Infected Patient.

The management of visceral leishmaniasis (VL) in HIV-infected patients is complex because of high mortality rates, toxic drug-related side effects, and a high risk of treatment failure and relapse. We report a case of active chronic VL in an HIV-1-infected woman presenting multiple secondary VL episodes over 7 years leading to massive splenomegaly and blood transfusion-dependent anemia despite several treatment courses and secondary prophylaxis. The patient was finally successfully treated with rescue treatment based on intravenous pentamidine.

T-L technique for HoLEP: perioperative outcomes of a large single-centre series.

Introduction: The aim of this article was to describe, step-by-step, an original technique (T-L technique) in a single centre series of patients who underwent holmium laser enucleation of the prostate (HoLEP) for symptomatic benign prostatic hyperplasia and analyze perioperative outcomes.

Material And Methods: We retrospectively analyzed data of 567 patients who underwent HoLEP. The T-L technique consists of a series of incisions used as landmarks, performed at the beginning of the procedure before enucleation.

Perioperative Outcomes of Holmium Laser Enucleation of the Prostate: A Systematic Review.

Introduction: The aim of the study was to systematically review the literature and describe perioperative complications of holmium laser enucleation of the prostate (HoLEP), including the Clavien-Dindo classification of surgical complications.

Methods: All English language publications on HoLEP were evaluated. We followed the PRISMA (Preferred Reporting Items for Systematic Review and Meta-Analyses) guidelines to evaluate PubMed®, Scopus®, and Web of Science™ databases from January 1, 1998, to June 1, 2020.

Edinburgh Lamarckians? The Authorship of Three Anonymous Papers (1826-1829).

In the space of four years, from 1826 to 1829, the Edinburgh New Philosophical Journal published three anonymous articles seemingly advocating doctrines inspired by Jean-Baptiste Lamarck. Decades of scholarship have initially attributed the most outspoken of the three articles, the 1826 "Observations on the Nature and Importance of Geology," to Robert Grant, and subsequently to Robert Jameson, thanks to a critical reassessment by James Secord (1991). More recently, scholars have also ascribed to Jameson an article published in 1829, "Of the Continuity of the Animal Kingdom by Means of Generation from the First Ages of the World to the Present Times.

The combination of waterjet ablation (Aquabeam) and holmium laser power for treatment of symptomatic benign prostatic hyperplasia: early functional results.

Introduction: The aim of this study was to assess the short-term functional outcomes and the efficacy of hemostasis performed with holmium laser performed following prostatic hydroablation with the Aquabeam system.

Material And Methods: Between June 2019 and July 2020, 53 consecutive patients underwent Aquabeam with our modified hemostasis approach with holmium laser. The following standard preoperative assessments were retrospectively recorded: prostate volume; International Prostate Symptom Score (IPSS) and Quality of Life (IPSS-QoL); uroflowmetry including Qmax and post void residual volume (PVR).

Coarse graining and adsorption in bottlebrush-colloid mixtures.

We study the adsorption properties in bottlebrush/colloid binary mixtures by combining scaling theories, theoretical predictions, self-consistent field computations (SCFC), and molecular dynamics simulations. In particular, we focus on adsorption in the case in which an attraction is set between the two species, by analysing the solution properties for a range of interactions and the size ratio between colloids and bottlebrushes, in the case in which colloids are smaller than the macromolecules. We show that the onset of adsorption is dominated by the local properties of the adsorbing guest particle.

Theoretical and Experimental Design of Heavy Metal-Mopping Magnetic Nanoparticles.

Herein, we show a comprehensive experimental, theoretical, and computational study aimed at designing macromolecules able to adsorb a cargo at the nanoscale. Specifically, we focus on the adsorption properties of star diblock copolymers, , macromolecules made by a number of diblock copolymer arms tethered on a central core; the monomeric heads, which are closer to the tethering point, are attractive toward a specific target, while the monomeric tails are neutral to the cargo. Experimentally, we exploited the adaptability of poly(2-oxazoline)s (POxs) to realize block copolymer-coated nanoparticles with a proper functionalization able to interact with heavy metals and show or exhibit a thermoresponsive behavior in aqueous solution.

Prevalence and factors associated with HIV-1 multi-drug resistance over the past two decades in the Italian ARCA database.

Despite successful antiretroviral therapy (ART), patients infected with human immunodeficiency virus (HIV) can develop multi-class drug resistance (MDR). This retrospective study aimed to explore the prevalence of HIV-1 drug resistance over the past two decades by focusing on HIV-MDR and its predictors. ART-experienced patients with HIV with results from at least one plasma genotypic resistance test (GRT) from 1998 to 2018, from the Antiviral Response Cohort Analysis database, were included in this study.

Adding systematic biopsy to magnetic resonance ultrasound fusion targeted biopsy of the prostate in men with previous negative biopsy or enrolled in active surveillance programs: A prospective single center, randomized study.

Magnetic resonance imaging (MRI) targeted biopsy (TBx) of the prostate demonstrated to improve detection rate (DR) of clinically significant prostate cancer (csPCa) in biopsy-naive patients achieving strong level of evidence. Nevertheless, the csPCa yield for TBx alone versus TBx plus systematic biopsy (SBx) after accounting for overlapping of SBx cores with TBx cores, in prior-negative or active surveillance (AS) patients has not been well established.The objective of the study was to investigate benefits in terms of detection rate and pathological stratification of prostate cancer (PCa) using contextual SBx during MRI-TBx.

Molecular and Cellular Substrates for the Friedreich Ataxia. Significance of Contactin Expression and of Antioxidant Administration.

In this study, the neural phenotype is explored in rodent models of the spinocerebellar disorder known as the Friedreich Ataxia (FA), which results from mutations within the gene encoding the Frataxin mitochondrial protein. For this, the M12 line, bearing a targeted mutation, which disrupts the Frataxin gene exon 4 was used, together with the M02 line, which, in addition, is hemizygous for the human Frataxin gene mutation (Pook transgene), implying the occurrence of 82-190 GAA repeats within its first intron. The mutant mice phenotype was compared to the one of wild type littermates in regions undergoing differential profiles of neurogenesis, including the cerebellar cortex and the spinal cord by using neuronal (β-tubulin) and glial (Glial Fibrillary Acidic Protein) markers as well as the Contactin 1 axonal glycoprotein, involved in neurite growth control.

The pathological and clinical features of anterior lesions of prostate cancer: Evaluation in a single cohort of patients.

Introduction: The aim of our work is to evaluate the principal differences of the pathological features in prostate cancer (PCa) lesions comparing those in the anterior region of the gland (APCa) to those in the posterior zone (PPCa) among patients who underwent to robotic-assisted radical prostatectomy (RP).

Material And Methods: A total of 85 consecutive patients (mean age 66; IQR 62-71) with clinically suspected PCa were studied with multiparametric magnetic resonance of prostate before prostate biopsies. The prostate biopsies were RM-guided (60 inbore biopsy (MR-GB) and 25 Fusion-biopsy (FB).
