Article Synopsis
  • Meniscal extrusion (ME) is a significant predictor of knee joint degeneration, but existing classifications primarily use numerical assessments without considering the underlying causes of ME.
  • Evidence suggests that ME can arise from various conditions, including acute tears, chronic degeneration, and even joint loading or anatomical issues that are not necessarily indicative of pathology.
  • A new qualitative classification is proposed to categorize ME into paraphysiological, pathological, and degenerative types, with an emphasis on adapting clinical management based on these distinct categories.

Purpose: Despite advances in surgical techniques and rehabilitation protocols, arthrofibrosis following total knee arthroplasty (TKA) still has poor outcomes. In the last decade, attention has been focused on the pathogenesis and cascade of events leading to the development of fibrosis. Currently, one of the most promising approaches consists in the indirect antagonisation of transforming growth factor beta 1 () through the downregulation of the renin-angiotensin-aldosterone system (RAAS).

Article Synopsis
  • Scientists are discovering that there are tiny microbes, like bacteria, in knee joints, even though we thought those places were clean before.
  • * A study looked at eight research papers that included 255 knees and found that these knee microbes exist, with detection rates between 5.8% and 100%.
  • * These microbes, especially from a group called Proteobacteria, seem to be more common in knees with osteoarthritis, suggesting they might be connected to the condition getting worse.

In recent years, there has been a significant focus on enhancing the overall quality of gluten-free breads by incorporating natural and healthy compounds to meet consumer expectations regarding texture, flavor, and nutritional value. Considering the high glycemic index associated with gluten-free products, the use of honey, renowned for its numerous health benefits, may serve as an optimal alternative to sucrose. This study investigates the impact of substituting sucrose, either partially (50%) or entirely (100%), with five Sardinian honeys (commercial multifloral honey, cardoon, eucalyptus, and strawberry tree unifloral honeys, and eucalyptus honeydew honey), on the rheological properties of the doughs and the physico-chemical and technological properties of the resulting gluten-free breads.

Article Synopsis
  • The study aimed to compare outcomes of cartilage repair in knee compartments using an aragonite-based scaffold versus standard surgical methods (debridement/microfractures) in 247 patients with knee lesions.
  • Results showed that the scaffold group had significantly better clinical and imaging outcomes at 24 and 48 months, including higher scores on subjective assessments (KOOS and IKDC) and better defect filling on MRI.
  • The conclusion indicated that the aragonite scaffold was safe and effective for treating knee cartilage defects, outperforming standard surgical care up to four years post-treatment.

The upcycling of agricultural by-products and the extension of the shelf-life of staple foods represent crucial strategies for mitigating the consequences of food losses and enhancing the competitiveness of the agri-food industry, thus facilitating the attainment of higher financial revenues. This is particularly relevant for global artichoke cultivation, where 60-80% of its biomass is discarded annually. The present study investigated the potential of using non-stabilized polyphenol-rich extracts from the main artichoke by-products (bracts, leaves, and stems) to fortify and extend the shelf-life of breadsticks.

Bone marrow lesions (BMLs) are MRI-visible subchondral bone alterations, highly correlated with symptoms in the knee. Subchondroplasty (SCP) is able to fill the subchondral defects associated with BMLs using an injectable bone substitute material. The aim of the present study is to evaluate the 12-month outcomes of the SCP in the treatment of symptoms of mild-to-moderate knee osteoarthritis (OA) patients with persistent BMLs of the knee.

Introduction: Osteoarthritis (OA) is a prevalent cause of disability worldwide, affecting millions and posing significant socioeconomic burdens. Various conservative measures like hyaluronic acid (HA) and platelet-rich plasma (PRP) injections aim to manage OA symptoms and delay surgical interventions. Despite the increasing utilization of PRP, consensus on its efficacy remains elusive, reflecting the evolving landscape of OA management.

Introduction: Advancements in monoclonal antibodies, tyrosine kinase inhibitors, and antibody drug conjugates (ADCs) have notably enhanced outcomes for metastatic HER2-positive breast cancer patients. Despite the expanding treatment options and clinical complexities, determining the optimal sequence of HER2-targeted therapies remains partly uncertain, influenced by various factors.

Methods: To refine HER2-positive metastatic breast cancer management, particularly regarding tucatinib's position, a Steering Committee of leading oncologists in breast cancer care devised a panel of statements via a Delphi approach, focusing on five key topics: general clinical management, therapeutic approaches for patients with HER2-positive breast cancer and brain metastases, treatment sequence, and tucatinib's safety and efficacy.
