Nowadays, sleepiness in patients with sleep apnea-hypopnea syndrome (SAHS) is understandable. It is somewhat more difficult to explain why most patients with SAHS enrolled in epidemiologic studies, even those with a high apnea-hypopnea index, do not experience excessive daytime sleepiness.
