Although rare, canal of Nuck hernias containing pelvic structures or peritoneal contents are an important differential consideration in the pediatric setting of a palpable labial mass. This diagnosis should be considered early as there is a high rate of associated ovarian torsion in the setting of incarcerated hernias containing ovaries. Early sonographic evaluation is crucial to establish the diagnosis.
