Introduction Post-endoscopic resection (ER) management of high-risk T1 esophageal adenocarcinoma (EAC) is debated, with conflicting reports on lymph node metastases (LNM) We aimed to assess outcomes following radical ER for high-risk T1 EAC. Methods We identified patients who underwent radical ER (tumor-negative deep margin) of high-risk T1 EAC, followed by surgery or endoscopic surveillance, between 2008-2019 across 11 international centers. Results In total, 106 patients (86 men, 70 ±11 years) were included.

Article Synopsis
  • Undifferentiated early gastric cancer (UD-EGC) can be treated effectively with endoscopic submucosal dissection (ESD), and this study focused on understanding recurrence rates and risk factors related to this treatment.
  • Data was gathered from 17 centers, involving 71 patients, indicating a low recurrence rate of 5.6% after an average follow-up period of about 29 months.
  • Factors like lymphovascular and perineural invasion were found to be significantly linked to local recurrence, while lesion size was not a predictor, suggesting that ESD is a viable initial treatment option for UD-EGC when certain risk factors are absent.

Saliva, bubbles, or mucus can limit gastric mucosal visualization (GMV), increasing the risk of missed lesions such as gastric cancer. Several studies using endoscopy photodocumentation-based scores have reported increased quality of GMV when mucolytic and/or defoaming agents are administered. This single-center, prospective, double-blind, randomized, placebo-controlled trial aimed to evaluate whether simethicone administration could improve GMV.
