Incident reporting and learning systems provide an opportunity to identify systemic vulnerabilities that contribute to incidents and potentially degrade quality. The narrative of an incident is intended to provide a clear, easy to understand description of an incident. Unclear, incomplete or poorly organized narratives compromise the ability to learn from them.

Established kilovoltage cone-beam computed tomography (kV-CBCT) image quality assurance (QA) guidelines often rely on recommendations provided by the American Association of Physicists in Medicine (AAPM) task group (TG) reports with metrics that use visual analysis. Methods: We analyzed the data of 108 patients (a total of 201 tumors) treated by GKRS with G-frame for head fixation. After co-registering the CBCT images and plan reference images, the Leksell GammaPlan (LGP) treatment planning system provided the amount of geometric translation and rotation required to minimize the position difference between the plan and treatment.

This paper analyses an area of 1152 000 km , between parallels 21 and 32°S and meridians 70 and 53°W to examine whether a large and taxonomically diverse data set actually displays areas supported by different groups.
