Article Synopsis
  • Chest pain is a common reason for emergency room visits, and this case discusses a 48-year-old woman who initially refused hospital admission despite presenting with typical chest pain suggesting a coronary issue.
  • During further evaluation with a CT coronary angiography, an esophageal tumor (leiomyoma) was accidentally discovered, which was surgically treated, leading to the patient becoming asymptomatic.
  • One year later, she complained of similar chest pain again; while it was initially thought to be related to the previous tumor, tests confirmed it was due to coronary issues this time, yet she still opted to avoid hospital admission for further procedures.

Article Synopsis
  • Upper gastrointestinal bleeding (UGIB) is a serious health issue that leads to significant patient risk and requires considerable healthcare resources, prompting a study on non-variceal bleeding severity related to certain medications.
  • The research analyzed 296 patients with UGIB at a hospital in Timisoara, categorizing them based on their use of drugs like NSAIDs, anticoagulants, and antiplatelet therapies, using statistical methods to assess the differences among groups.
  • Results indicated that patients taking NOACs experienced more severe bleeding and worse blood parameters than those on vitamin K antagonists (VKAs), contradicting existing studies and highlighting the potential iatrogenic risks associated with these medications.

Article Synopsis
  • The prevalence of atrial fibrillation (AF) has increased over the past 20 years, making it the most common heart rhythm disorder, thus requiring more in-hospital management for patients experiencing acute issues.
  • Research indicates that a higher neutrophil-to-lymphocyte ratio (NLR) can predict in-hospital mortality, suggesting it could serve as an important biomarker for patient outcomes.
  • A study at a medical clinic analyzed 1111 patients admitted for AF and found that those who died had a significantly higher NLR at admission compared to survivors, confirming that NLR is an independent predictor of in-hospital death.

Article Synopsis
  • Sinusoidal obstruction syndrome/veno-occlusive disease (SOS/VOD) is a rare but serious liver condition often linked to chemotherapy and bone marrow transplants, characterized by signs of portal hypertension.
  • The case discusses a 65-year-old woman who experienced symptoms like fatigue, abdominal tenderness, and elevated liver enzymes after undergoing chemotherapy for colorectal cancer with liver metastases.
  • Despite the diagnosis being complicated by her recent endometrial cancer treatment, timely identification of SOS/VOD is crucial for managing risks and improving patient outcomes.

Article Synopsis
  • A study involving 300 participants, including dyslipidemic patients on and off statins and healthy controls, aimed to explore the connection between long-term statin use, gallstone disease (GSD), and gut microbiota differences.
  • Results showed that gut dysbiosis was more prevalent in statin-negative patients, and GSD was less common in those on statins, suggesting a protective effect.
  • Significant correlations were found between GSD and conditions like obesity and diabetes, with gut dysbiosis and obesity identified as independent risk factors for GSD among statin patients.

Hepatocellular carcinoma (HCC) is the most frequently found primary malignancy of the liver, showing an accelerated upward trend over the past few years and exhibiting an increasing relationship with metabolic syndrome, obesity, dyslipidemia and type 2 diabetes mellitus. The connection between these risk factors and the occurrence of HCC is represented by the occurrence of non-alcoholic fatty liver disease (NAFLD) which later, based on genetic predisposition and various triggers (including the presence of chronic inflammation and changes in the intestinal microbiome), may evolve into HCC. HCC in many cases is diagnosed at an advanced stage and can be an incidental finding.

Purpose: This study aimed to explore inflammatory biomarkers, stool's functional bacterial groups and their possible link to portal vein thrombosis (PVT) in patients with liver cirrhosis (LC).

Materials And Methods: An observational study of 300 participants: 200 inhospital cirrhotic patients, who met inclusion criteria, equally assigned into two groups, based on the presence or absence of PVT and 100 healthy controls was carried out.

Results: The PVT group displayed significant differences related to older age, cigarettes smoking history, emergency admission, higher Child-Pugh score, metabolic related disorders and nonalcoholic fatty liver disease, as well as non-obstructive aspects, with chronic thrombi.

Article Synopsis
  • The study explored how metabolic syndrome (MetSy) is related to neuropsychic complications in patients with atrial fibrillation (AF), comparing those with MetSy to those without.
  • It found that AF patients with MetSy experienced fewer cases of cognitive impairment and dementia, suggesting a possible protective effect of MetSy against these issues.
  • Among the MetSy components, obesity was highlighted as the most significant factor providing this protective effect, while the differences in stroke and Parkinson's disease occurrence were not statistically meaningful.

Current data indicate the existence of post-acute COVID-19 syndrome frequently expressing as cardiovascular and respiratory health issues. The long-term evolution of these complications is not yet fully known or predictable. Among the most common clinical manifestations of post-acute COVID-19 syndrome are dyspnea, palpitations, and fatigue, in most cases being transient and without underlying any morphological or functional changes.

Article Synopsis
  • - Atrial fibrillation (AF) is growing in prevalence and is associated with a higher risk of serious brain complications like stroke and dementia, especially in patients with permanent AF compared to those with paroxysmal AF.
  • - A study of 1111 AF patients revealed significant differences in complications: 48.75% of permanent AF patients had strokes versus 26.74% of paroxysmal AF patients, and dementia rates were also higher in the permanent group.
  • - The research concluded that permanent AF patients face greater risks for cardio-embolic complications, emphasizing the need for prompt treatment, especially as paroxysmal AF often affects younger individuals.

Objectives: COVID-19 has varied clinical manifestations, from asymptomatic to severe cases, and conjunctivitis is one of them, but sometimes a lone initial symptom is found to be present. The aim of this study was to identify the prevalence of conjunctivitis as the first symptom in COVID-19 patients in a primary healthcare unit.

Methodology: A retrospective study was conducted, analyzing the presenting complains/symptoms and results of COVID-19-confirmatory tests.

Background: Poorly informed college students tend to adopt the habit of cigarette smoking. This habit often continues into their adulthoods, adversely affecting the population's health and increasing the burden on healthcare systems.

Aim: We aimed at exploring the predictors of the avoidable habit of smoking.

Introduction: The role of digoxin (cardiac glycoside) in controlling the heart rate (HR) for the treatment of atrial fibrillation (AF) patients has not been explored in depth.

Methods: To contribute to the limited data, our team conducted retrospective analysis of the clinical records of 1444 AF patients. We divided the AF patients into two groups, wherein group 1 patients were treated with beta-blockers (BB), low-dose digoxin, and an anticoagulant (vitamin K antagonist/factor-IIa inhibitor/factor-Xa inhibitor), and group 2 patients were treated with just BB and an anticoagulant.
