Background: The acute and chronic pancreatitis (CP) can lead to severe complications like walled-off necrosis, large symptomatic pseudocyst or multiorgan failure. The treatment of these complications is multivariate and can differ from conservative, symptomatic treatment or minimal-invasive, endoscopic transgastral stenting to transgastral necrosectomy.

Objectives: This study aims to analyse the clinical course for patients that develop local complications of severe pancreatitis.

Background: Endoscopic retrograde cholangiopancreaticography (ERCP) is the standard endoscopic procedure for the diagnosis and treatment of diseases of the pancreas and bile ducts. Cholangioscopy provides direct visualization of the bile ducts. It offers the possibility of more detailed diagnostic and therapeutic indications.

The case of a 22-year-old male patient who presented with acute on chronic hyperglycemia in known MODY ("maturity onset diabetes of the young") 12 (ABCC8 gene) after 11 months of treatment cessation is reported. To emphasize the importance of the awareness of this therapeutically important entity of diabetes, the essential facts of this inherited disease are summarized.

The fungicide fluxapyroxad (BAS 700 F) has been shown to significantly increase the incidence of liver tumours in male Wistar rats at dietary levels of 1500 and 3000 ppm and in female rats at a dietary level of 3000 ppm via a non-genotoxic mechanism. In order to elucidate the mode of action (MOA) for fluxapyroxad-induced rat liver tumour formation a series of in vivo and in vitro investigative studies were undertaken. The treatment of male and female Wistar rats with diets containing 0 (control), 50, 250, 1500 and 3000 ppm fluxapyroxad for 1, 3, 7 and 14 days resulted in a dose-dependent increases in relative weight at 1500 and 3000 ppm from day 3 onwards in both sexes, with an increase in relative liver weight being also observed in male rats given 250 ppm fluxapyroxad for 14 days.

Article Synopsis
  • SARS-CoV-2 infection, which causes COVID-19, can lead to severe illness and multiple organ failure, potentially resulting in complications like secondary sclerosing cholangitis in patients recovering from the virus.
  • A study of 249 hospitalized patients during the first wave of the pandemic found that 35.3% needed intensive care, with a 7.8% incidence of confirmed SSC-COVID among those followed up; all affected patients had prolonged ICU stays.
  • The findings suggest that SSC-COVID is a notable post-COVID complication and should be considered in cases of ongoing unclear liver-related symptoms following SARS-CoV-2 infection.

Background And Aims: Postpolypectomy syndrome (PPS) is a relevant adverse event that can appear after polypectomy. Several publications mention postpolypectomy syndrome using different criteria to define it. The aim of this study is to detect potential risk factors and predictors for developing PPS and to define the main criteria of PPS.

Article Synopsis
  • About 60% of pharmacological randomized trials use standard placebos, which may not effectively mask the non-therapeutic side effects of experimental drugs, leading to possible unblinding of participants.
  • The study aimed to compare drug effects when using active placebos—designed to mimic side effects—against standard placebos to assess whether the latter may overestimate drug effectiveness.
  • The researchers conducted a thorough search of various databases and selected trials that compared active versus standard placebos, carefully analyzing data while considering biases and categorizing active placebos based on their effects.

Endoscopic negative pressure therapy is an effective treatment strategy for various defects of the gastrointestinal tract. The functional principle is based on an open-pore element, which is placed around a perforated drainage tube and connected to a vacuum source. The resulting open-pore suction device can undergo endoluminal or intracavitary placement.

Article Synopsis
  • * A study examined 14 patients who underwent endoscopic treatment for leaks, finding differences in treatment outcomes between those who received a stent and those who had negative pressure therapy.
  • * The stent group had fewer endoscopies, a shorter hospital stay, and quicker resumption of oral nutrition, leading to higher success rates compared to the negative pressure group, showing stent therapy is more effective.

Background: Since December 2019, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has posed a pandemic threat to global health. We are now in the fourth wave of this pandemic. As the pandemic developed, the requirements and therapeutic endoscopic procedures for SARS-CoV-2-positive patients underwent changes.

Background: Esophageal cancer (EC) is the sixth-leading cause of cancer-related deaths in the world. Esophagectomy is the most effective treatment for patients without invasion of adjacent organs or distant metastasis. Complications and relevant problems may occur in the early post-operative course or in a delayed fashion.

Background: After intestinal transplantation, close allograft monitoring especially during the early postoperative period is crucial since the intestine is a highly immunogenic organ. Current protocols are based on endoscopic and histologic examination with the latter one being linked to the risk of bleeding and perforation.

Aims: Evaluation of the diagnostic value of endoscopy utilizing magnification to predict acute cellular rejection compared to routine allograft biopsies.

Article Synopsis
  • - The text discusses the use of endoscopic negative pressure therapy as a treatment for gastrointestinal leaks, particularly focusing on its application for rare and dangerous duodenal diverticular perforations.
  • - Two cases are presented where patients diagnosed with perforated duodenal diverticula were successfully treated with this therapy alone, showing effective results despite initial inconclusive symptoms.
  • - The authors emphasize the potential of endoscopic negative pressure therapy as an alternative to surgical intervention for duodenal diverticular perforations, suggesting it should be considered as the first line of treatment.

Healing of gastrointestinal ulcers after Hemospray application was reported in literature. The pathophysiological mechanism of action of hemostatic powders is not elucidated so far. A prospective animal model was performed to evaluate the effect of Hemospray application on the healing process of artificially induced ulcers of the upper and lower gastrointestinal tract.

Alcohol abuse after liver transplantation can seriously impact graft and patient survival. However, to date, there is no defined standard procedure to identify patients consuming alcohol after liver transplantation. The aim of this study was to analyze the diagnostic value and clinical impact of routinely measured urinary ethyl glucuronide (uEtG) - a metabolite of ethanol - in patients after liver transplantation.

Background And Study Aims: Since December 2019, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the causative pathogen of coronavirus disease 2019 (COVID-19), has posed a pandemic threat to global health and has challenged health care system in all affected countries.

Patients And Methods: This is a combined study including a descriptive part about the changes in the daily work routine of an Interdisciplinary Endoscopic Unit (IEU) and a prospective analysis of patients tested positive for SARS-CoV-2 who required endoscopic interventions. Conclusively, we present the finding of a point-prevalence analysis in the staff of the IEU.

Aims: Prevention and prediction of microvascular complications are important aims of medical care in people with type 1 diabetes. Since the course of the disease is heterogenous, we tried to identify subgroups with specific risk profiles for microvascular complications.

Methods: Retrospective analysis of a cohort of 285 people (22637 consultations) with >10 years of type 1 diabetes.

Background: Prostate cancer (PCa) is the most common type of cancer among men in Western countries. Despite numerous therapeutic options, few treatments are available for patients with end-stage disease. In the present study, different somatostatin receptors (SSTs) and the chemokine receptor CXCR4 were evaluated for their suitability as novel therapeutic targets in PCa.
