"Gravity" - a new simple negative pressure wound therapy self-build design for low income countries.

Negative Pressure Wound Therapy (NPWT) is prohibitively expensive at the moment and therefore not easily accessible in low-income countries. An additional problem is the requirement of consistent reliable electricity to power the devices. A new low-cost NPWT device was designed from low cost and simple materials and it can be built and operated following a simple set of instructions.

Revision knee arthroplasty using a distal femoral replacement prosthesis for periprosthetic fractures in elderly patients.

Purpose: Distal femoral periprosthetic fractures above a total knee replacement in elderly patients are technically challenging to treat. Bone quality is often poor, the fractures comminuted, and post-operative mobilisation is difficult. This study assesses the clinical, radiological and functional outcome of revision knee distal femoral replacement (DFR) in these fractures.

Combined femoral and sciatic nerve block vs combined femoral and periarticular infiltration in total knee arthroplasty: a randomized controlled trial.

This study tests the null hypothesis that there is no difference between sciatic nerve block (SNB) and periarticular anesthetic infiltration (PI) as adjuncts to femoral nerve blockade (FNB) in total knee arthroplasty in terms of postoperative opioid requirements. Fifty-two patients undergoing total knee arthroplasty were randomized to receive either (a) combined FNB-SNB or (b) combined FNB-PI. Average morphine consumption in the first 24 (20 vs 23 mg) and 48 hours (26 vs 33 mg) showed no significant difference.

Large Osteochondral Fracture in a Patient with Bilateral Bicondylar Osteochondritis Dissecans and Spondyloepiphyseal Dysplasia Tarda.

We report a case of a large osteochondral fracture in a patient with bicondylar, bilateral osteochondritis dissecans and spondyloepiphyseal dysplasia tarda.
